Job summary

Cape Hill Medical Centre has an exciting opportunity for an ANP to join our friendly, hardworking and patient focused practice team. We are looking for a team player with the enthusiasm to make a difference to primary care.

We are a training practice, with 4 Clinical partners, 6 salaried GPs, 4 ANP, 2 Practice Nurses and 1 HCA. The entire clinical team is supported by a fully staffed admin and reception team allowing us to provide excellent patient care.

We are part of CARITAS PCN with another 2 local Practices with a cohort of additional roles staff support to share and deliver clinical care to ensure the patients see the right clinician for their needs

Main duties of the job

  • An experienced ANP resource for health care professionals and service users, working in collaboration with other members of our multi-disciplinary team.
  • Assess, diagnose, treat, refer or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.
  • You will hold experience and skills to be able to assess, diagnose, treat and refer patients with a range of chronic diseases and / or specialist support e.g. diabetes, asthma, heart failure, rheumatology.
  • Use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment.
  • An experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
  • You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice
  • You will demonstrate critical thinking in the clinical decision-making process.

  • Work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Job description

Job responsibilities

The following are the core responsibilities of the advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

b. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients

c. Implement and evaluate individual specialised treatment plans for chronic disease patients

d. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

e. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

f. Provide wound care (ulcer /Doppler etc.) to patients

g. Provide specialist clinics such as ear syringing, contraception etc.

h. Undertake the collection of pathological specimens

i. Provide travel medicine services

j. Request pathology services as necessary

k. Process and interpret pathology and other test results as required

l. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

m. Maintain accurate clinical records in conjunction with extant legislation

n. Ensure read codes are used effectively

o. Maintain chronic disease registers

p. Be responsible for the implementation of an effective immunisation programme

q. Chaperone patients where necessary

r. Prioritise health issues and intervene appropriately

s. Support the team in dealing with clinical emergencies

t. Recognise, assess and refer patients presenting with mental health needs

u. Implement vaccination programmes for adults and children

v. Extended and supplementary prescriber, adhering to extant guidance

w. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

x. Contribute to practice targets (QOF etc.), complying with local and regional guidance

y. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

z. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

aa. Support the clinical team with all safeguarding matters, in accordance with local and national policies

bb. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
