Job summary
Cape Hill Medical Centre has an exciting opportunity for an ANP to join our friendly, hardworking and patient focused practice team. We are looking for a team player with the enthusiasm to make a difference to primary care.
We are a training practice, with 4 Clinical partners, 6 salaried GPs, 4 ANP, 2 Practice Nurses and 1 HCA. The entire clinical team is supported by a fully staffed admin and reception team allowing us to provide excellent patient care.
We are part of CARITAS PCN with another 2 local Practices with a cohort of additional roles staff support to share and deliver clinical care to ensure the patients see the right clinician for their needs
Main duties of the job
- An experienced ANP resource for health care professionals and service users, working in collaboration with other members of our multi-disciplinary team.
- Assess, diagnose, treat, refer or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.
- You will hold experience and skills to be able to assess, diagnose, treat and refer patients with a range of chronic diseases and / or specialist support e.g. diabetes, asthma, heart failure, rheumatology.
- Use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment.
- An experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
- You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice
- You will demonstrate critical thinking in the clinical decision-making process.
- Work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
