JOB DESCRIPTION - PRACTICE NURSE
Job Purpose
To be
part of the nursing team responsible for providing a high quality nursing
service to the practice population.
To
ensure optimum levels of nursing care is given to all patients.
To make
the best use of clinical skills within the practice to enable the provision
of a diverse range of services within the competence of the post holder.
Role and Responsibilities
Provide
nursing treatments to patients in participation with general practitioners or
independently to agreed protocols
Advise
patients on general health care and specific health screenings with referral
to GPs as necessary
Meeting
the nursing care needs of patients as identified or transferred to practice
nurse care by the GPs or Advanced nurse practitioner
Monitor
& manage the care of patients with: - Asthma Diabetes
Hypertension IHD
COPD using recall facilities on the clinical system
LES/DES/QOF
Meet the criteria required for related clinical and administrative work
Cervical
cytology & teaching breast awareness
Contraception
Swabs
Wound
care / ulcer care / Compression Bandaging
Treatment
of minor injuries/ Removal of Sutures
New
Patient Medicals / Health checks/ Urinalysis / BP checks
ECGs
COPD
/ Asthma / Peak Flow Readings / Spirometry
Venipuncture
Ear
syringing
Pneumococcal,
shingles and annual flu
immunisations
Travel Clinic providing accurate, up to date travel advice and
where appropriate immunization and malaria prophylaxis for registered
patients with the practice
Any
other tasks/delegated duties appropriate to the post or requested by the partners or
practice manager
Extended
responsibilities
1.
Supplies
and equipment Treatment room and other areas. Ordering
of vaccinations and equipment to maintain stock levels.
2.
Pathological
specimens and investigatory procedures.
Undertake
the collection, storage and dispatch of pathological specimens. Requesting
pathology tests.
3.
Administrative
and professional responsibilities.
Ensure
accurate notes of all consultations and treatments are recorded on the clinical computer system as appropriate.
Assist
in the formulation of Practice philosophy, strategy and policy and develop appropriate
protocols.
Attend
and participate in any Practice meetings when required.
Quality
The post-holder will strive to maintain quality
within the Practice.

An
understanding, acceptance and adherence to the need for strict
confidentiality.
Safeguarding Children and Adults
The practice is
committed to safeguarding and promoting the welfare of children, young people
(anyone under the age of 18) and
adults.
Staff are
expected to know about and follow the practice safeguarding children and
safeguarding adult policies.
Health &
Safety & Risk Management
The post-holder will assist
in promoting and maintaining their own and others health, safety and security
as defined in the Practice Health & Safety Policy.
Using
appropriate infection control procedures, maintaining work areas in a tidy
and safe way and free from hazards, reporting
potential risks identified.
Keep up to date with Health &
Safety procedures, policies & guidelines.

The post-holder should

Personal/Professional Development
The post-holder will participate in any training
program implemented by the Practice as part of this employment.
Participation
in an annual individual performance review and taking responsibility
for maintaining a record of your own personal and/or professional development.
Maintain
own annual registration with the NMC; adhere to the Code of professional
conduct and complete the necessary work for the three-yearly re-validation.
Complete
eLearning training as per Practice schedule.
Basic
Life Support and Defibrillator training.
Membership
of the RCN / NMC.

Ability

up to date with all aspects of nursing care relevant to the post.

commitment to ensure all Health and Safety requirements and Infection Control
measures are met and to report any problems to the Practice Manager.

An
awareness of own limitations and experience.
To
work only in accordance with the UKCC Code of Conduct and within the scope of
Professional Practice.
To
have a written professional development plan and to maintain an up to date portfolio
which meets the requirements of registration with the UKCC.

Qualifications
and Education Requirements.
Registered
General Nurse with proven evidence of appropriate knowledge base.
Practice
Nurse background preferable but not essential.
Currently
working with or an interest in Diabetes and/or respiratory disease would be
desirable.
Preferred
Skills
Above
average interpersonal skills with ability to communicate effectively both
verbally and in writing.
Good
team player.
Evidence
of problem solving and decision making skills.
Ability
to be reflective and accept constructive criticism where appropriate.
Able
to manage and adapt to change.
