Job summary

We would like an experienced Clinical Practitioner to come and join our friendly enthusiastic team. A supportive multi-disciplinary team that enables you to do what you do best; care for your patients with an increasing list size of 13,314 patients.

Based in a new modern/purpose built GP Surgery within the Northfields Health Village, Stafford. The Practice has an established reputation for GP training, Medical Students and Research Studies.

Main duties of the job

Duties will include acute on the same day face to face treatment management and working collaboratively with our Primary Care Team. The ability to work independently and to demonstrate critical thinking in the clinical decision-making process is required and to take lead responsibility for the care of patients.

Job description

Job responsibilities

Work autonomously and be accountable for his/her own professional actions. Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.

Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow up.

Discuss with a GP, where appropriate, make referrals to other health professionals in primary and secondary care.

Undertake acute home visits, arrange hospital admission for appropriate cases.

Maintain accurate record keeping in EMIS Web (Clinical System).

Request and interpret tests appropriate to patients needs.
