Job summary

We are looking for an experienced Practice Nurse to join our friendly and supportive team at Manor House Surgery.

Main duties of the job

Within the role of the Practice Nurse, you will provide care and management of patients with long term health conditions such as Asthma/COPD and Diabetes, as well as providing all aspects of nursing care duties such as wound care, adult and childhood immunisations, travel vaccinations, cervical cytology screening, contraception and promoting a healthy lifestyle.

About us

We are a professional and supportive team of GPs, Nurse Practitioners, Practice Nurses and Health Care Assistants who work alongside a friendly and dedicated admin/reception team. We also work alongside a Physicians Associate, Practice Pharmacist, Mental Health Nurse, Admiral Nurse (Dementia), Podiatrist, Physiotherapist and Health and Wellbeing Coach through our PCN. You will be supported by the whole team at Manor House.

We are based in Normanby in a Grade II listed Manor House. We are accessible via public transport and have free onsite parking which is monitored by security cameras.

Job responsibilities

Duties and Responsibilities include:

  • Managing long term conditions.
  • Working collaboratively with general practitioners to provide the highest level of care.
  • Assessing, evaluating and monitoring treatment plans.
  • Be competent and up to date with Cervical Cytology screening, childhood immunisations, contraception management and all aspects of immunisations and vaccinations.
  • Be proactive in promoting a healthy lifestyle and care navigate to appropriate services.
  • Mentor and support health care assistants.
  • Advise on foreign travel and administering travel vaccinations.
  • Managing complex pressure dressings and wound care management.
  • Ensure infections control procedures and policies are followed and kept current.
  • Participate in practice audits, development plans and meetings.
  • Maintain own annual registration with the NMC and adhering to Code of Professional Conduct.
  • Promote own continuous professional development.
  • Any other reasonable duties that may be required on occasion.

  • Current NMC Registration

Desirable

  • Experience of working in Primary Care is preferable.
  • Accredited training in chronic disease management
  • Qualification in COPD/Asthma management including spirometry interpretation.

  • Childhood immunisations
  • Compression Dressing and wound management
