Job summary
To act as the single point of administrative and
coordination support for the Older Persons Integrated Care (OPIC) Team within
the Barnstaple Alliance Primary Care Network. This role, alongside the OPIC Team clinicians, will work closely with the member practices to provide care to our care home and housebound patients registered within the PCN.
Hours available are 26hrs to 30 hrs a week, worked over 4 or 5 days.
Interviews planned for 10th March 2026.
Main duties of the job
The OPIC Team consists of Nurse Practitioners and a GP Clinical Lead.The PCN care co-ordinator role will
be pivotal in ensuring all patients receive the best possible care and service
through coordinating and supporting the delivery of all key activities of the OPIC
Team. This includes access to services, advice and information, ensuring a
patient centred approach with timely and effective care planning and
organisation.
The role will involve
co-ordination of OPIC home visits for older people living in their own homes
and those living in care homes. This will involve identifying patients for
visits, making bookings, preparing visit paperwork and entering data on to the
patient record, following visits from the OPIC clinicians. There will also be a role in vaccine planning
and coordination for the PCN. The role
will also include supporting digital initiatives.

- Provide efficient
and comprehensive administrative and clerical support to the OPIC Team on a
day-to-day basis.
- Coordinate and
arrange Annual Review visits and Proactive Care visits for the OPIC team, identifying
patients for these visits, contacting patients to make bookings, preparing
visit paperwork and inputting data after visits.

Continuously
monitor, manage, and prioritise all incoming and outgoing correspondence and
workflow for the OPIC Team (such as the OPIC teams email inbox and TPP
SystmOne task boxes), initiating responses where appropriate.
- Being the single
point of contact for the OPIC team for the member practices and teams within
the Barnstaple Alliance PCN, Care Homes and other healthcare professionals.
- Support the OPIC
Team with inputting data and maintaining patient records and patient lists.
- Day to day
support to the OPIC nurses- preparing paperwork for visits, printing blood
labels, supporting with admin.
- Practical
application of TPP SystmOne and Microsoft Teams for coordinating workload,
organising individual team members caseloads and appointments, and developing
reporting mechanisms.
- Coordinate
agendas, prepare patient lists and take minutes of key meetings including relevant
Multidisciplinary Team meetings.
- Produce via TPP
SystmOne and other digital tools reports to support the work of the team.

the Network Contract Directed Enhanced Service (DES). Including the Enhanced
Health in Care Homes (EHCH) and Structured Medication Review (SMR) areas.

patients into the Weekly Care Home Round appointment template within TPP
SystmOne as needed.
- To help Care Home
Managers and GP Practices address their patients needs through answering
queries, managing diaries and appointments, and delivering care plans.
- To support the
OPIC Team in the development of robust care plans in line with best practice.
- Organising which
elements of clinical (and QoF) checks need applying on visits.
- Support
and coordinate work around vaccinations for the PCN as needed (including
reporting, invitations, vaccine collection and data input).
