Job summary
Barnstaple Alliance Primary Care Network are looking for an enthusiastic and organised prescribing co-ordinator to join our Pharmacy team. You will be joining the current team which consists of Clinical Pharmacists and Pharmacy technicians.
The hours available are between 21- 27 hrs per week. In your application, please state your ideal number of hours you would like to work

The post holder will work
alongside an established team of Pharmacists and Pharmacy Technicians to provide administrative and coordination support to
the pharmacy team.

supportive network of four General Practitioner surgeries: Brannam, Fremington,
Litchdon and Queens Medical Centres. Between us we provide care to over 52,000
patients in the Barnstaple area. Whilst
retaining our independent surgeries, we are working together on collaborative
projects to improve and develop the healthcare services we offer to patients.

Barnstaple Alliance PCN Care Co-ordinator Responsibilities
- Carry out
audits to identify improvements and maximise income for prescribing IIF
(Investment and Impact fund) indicators.

Update IT
systems to include templates and protocols in order to meet the requirements of
any changes in service specifications, e.g. IIF changes.

Design
efficient templates, protocols, alerts and other IT tools to support drug
safety, patient care, quality record-keeping and reporting claims.
- work across
Practices to share good Practice, identify information and IT needs, and
implement effective systems and training. The aim is for all Practices to be
best of the collaborative group by sharing what is does well.
- Establish
good working relationships with people employed in Practices across the PCN to
enable them to carry out their duties effectively. It is important that the
skills of existing teams continue to be valued and their roles developed as
agreed with the Practice.
- Work with
the Clinical Pharmacists and Pharmacy technicians as well as other primary care
professionals within the PCN to identify and manage a caseload of patients, and
where required and as appropriate, refer people back to other health
professionals within the PCN.
- Communicate
with community pharmacy colleagues to assist Clinical Pharmacists and Pharmacy technicians.

Give
specific support to practices to help plan and deliver efficient vaccination
campaigns such as Flu and Covid 19 using IT tools available to you.
- Take
minutes of MDT meetings and disseminate; chase progress against actions
identified in these meetings and ensure follow up where necessary.
