Job summary

We are looking for a passionate and enthusiastic Wellbeing Linkworker to join our Primary Care Network (PCN) to provide personalised support to our patients across our member practices and Queens House EX32 8HJ.

Hours available are between 28 - 37 per week. Actual working hours will be agreed with the successful applicant. Please specify within your application your preferred working hours and pattern.

Interviews planned for 16th January 2025.

Main duties of the job

The PCN Wellbeing Linkworker is a patient-facing, non-clinical role whose core duties are to support patients to take proactive steps to improve the way they manage their physical and mental health, based on what matters to them. They will work holistically and deliver patient centred care.

This role will manage a patient caseload and work alongside Wellbeing Linkworkers, a Wellbeing Connector, a Coordinator and Team Lead as part of the PCNs wider Wellbeing Service. The role will also play an important part with service and community group engagement and networking.

The Barnstaple Alliance PCN is a proactive and supportive network of four General Practitioner surgeries: Brannam, Fremington, Litchdon and Queens Medical Centres. Between us we provide care to over 52,000 patients in the Barnstaple area. Whilst retaining our independent surgeries, we are working together on collaborative projects to improve and develop the healthcare services we offer to patients.

Job responsibilities

KEY RESPONSIBILITIES

  • To provide personalised support to patients, either by phone, face to face or video call, based on what matters to them, with the aim of: improving patients knowledge, skills, and confidence to empower them to take control of their wellbeing and improve their health outcomes.
  • Take a holistic approach, based on the persons priorities and the wider determinants of health.
  • Co-produce personalised support plans to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
  • To manage and prioritise a caseload, in accordance with the needs and priorities of the patient and liaise with other healthcare professionals within the PCN when the person's needs are beyond the scope of the wider Wellbeing Team.
  • Ensure affective engagement with the patient to build confidence and resilience to manage their own needs.
  • Draw on and utilise the strengths and capacities of local community groups and services to enable local VCSE to receive patient referrals from the PCN. organisations and community groups.
  • Support the Wellbeing Team to build and maintain a network or providers who will support patients as part of the Wellbeing Service.
  • Develop an understanding and promote the benefits of nature based (green and blue spaces) and outdoor activities to patients and build knowledge of these areas North Devon wide.
  • Effectively monitor, measure and evaluate the progress of patients who are active the teams caseload.

SERVICE RESPONSIBILTIES

  • Deliver an effective service whilst adhering to the requirements and boundaries of the PCN service to meet the needs of the member practices and patient population, led by the PCN and wider contractual requirements.
  • Be competent with the use of IT and patient record systems, ensuring you have the fundamental skills to work effectively on a computer, using basic troubleshooting techniques and appropriately raising issues with colleagues.
  • Update IT systems in a timely manner to ensure records (patient medical records and appointment ledgers) are accurate and consistent.
  • To promote and raise awareness of the Wellbeing Service particularly to practices within the PCN.
  • To develop strong working relationships with GPs, practice teams, PCN teams and other organisations to ensure a person-centred approach is provided and reduce duplication of resources.
  • Engage, contribute and on occasion and where agreed, lead on PCN initiatives, projects or pilots including but not limited to health inequalities and population health management.
  • To attend and contribute to Wellbeing Team meetings, Multi-disciplinary Meetings and events as required by the service.
  • On occasion, represent the Wellbeing Team and wider PCN in a professional manner at meetings or events required by the service.
  • To work flexibly, adapting to the needs of the service whilst maintaining the integrity of the role.
  • To participate in regular supervision and continual personal development.
