Main duties of the job

You will work under the indiract supervision of the Nurse Manager, providing leadership to the Nurse Team, delivering an effective and quality driven nursing service to Practice patients. You will supervise and monitor the performance of the Nurse Team, the systems and processes required for the team to function efficiently and delegate duties where appropriate.

You will have responsbility for the provision of skilled nursing care to the practice population, with an emphasis on the planning and co-ordinaton on the delivery of long-term condition care and QOF. As part of the role, you will continously evaluate and monitor the success of the service and will work collaboratively with other practice-based teams, local services, primary and secondary care to meet the needs of the patients.

As an autonomous practitioner you are responsbile for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You must always work within your own competence and use your own judgement, knowledge, and common sense in your day-to-day activities.
