Job summary

We are looking for full or part time Practice Nurse to join our team at Swanscombe Health Centre and branch practices.

Based at Swanscombe Health Centre, the role of the Practice nurse will be to work within our nursing team, to offer support and treatment for our patients, focusing particularly on health improvement. You should have experience of working in a similar role, with relevant qualifications, NMC registration, and have excellent organisational skills.

Main duties of the job

Duties will include (but not exhaustive);

  • Baby/Childhood immunisations
  • Injections
  • Wound care
  • Cervical Screening
  • Pill Checks/contraceptive clinics
  • Long term condition checks/support.
  • General PN duties

About us

We have a team of over 100 staff throughout 7 sites, including GPs, Paramedic Practitioner, Clinical Pharmacists, Physicians Associates, a Physiotherapist, Practice Nurses, Healthcare Assistants, Social Prescribers and the administrative support workers. Even with a large team, we work closely together and clinical staff are supported by a weekly clinical meeting.

Job description

Job responsibilities

The main duties and responsibilities are as follows.

1. Meeting patients nursing care needs as identified, or where care has been transferred to them by a GP. This will usually be in surgery but occasional home visits will also be required.

2. Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.

3. Managing the nursing care of patients, employing personal recall systems suitable to the practice needs. This includes using the practice computer system, EMIS, for input of information for recall and screening purposes. Accurate and efficient input of data.

4. The following clinics:

Holiday Vaccination Clinic

Flu Vaccination Clinic

Cervical Smears

Family Planning

Baby immunisation Clinic

Wound Management

INR star

Assisting GP with Minor Operations

Amending or adding to services as needs are identified

5. Maintaining efficient liaison with all areas of the Primary Care Team and other agencies to assure continuity of care where necessary.

6. Routine treatment in general clinics, including:

Warfarin monitoring

Wound management

Removal of foreign bodies

ECGs

Ear syringe

And other general Treatment Room skills depending on the service need
