Job summary
We are looking for full or part time Lead Practice Nurse to join our team at Swanscombe Health Centre and branch practices.
Based at Swanscombe Health Centre, the role of the Lead Practice nurse will be to work within and lead/manage our nursing team, be the IPC lead, to offer support and treatment for our patients, focusing particularly on health improvement. You should have experience of working in a similar role, with relevant qualifications, NMC registration, and have excellent organisational skills.
Main duties of the job
Duties will include (but not exhaustive);
- Managing a team of 5 nurses and 7HCAs
- Be the IPC Lead - completing audits, training etc
- Supporting trainees and team training
- Nursing team rotas
- Take part in MDT meetings inhouse
- General PN duties

The Lead Practice Nurse will be
responsible for several clinical areas such as infection prevention and control, health
promotion, chronic disease management, health prevention, as well as supporting the practice management
team in the reviewing and delivery of clinical policy and procedure.
The Lead Practice Nurse will be
responsible for the implementation of processes for the effective management of
patients with long-term conditions using evidence-based practice including care
for elderly and housebound patients.
Working as a senior member of the
practice multidisciplinary team, the post holder will ensure nursing services
are delivered effectively to the entitled patient population.
