This is a new permanent full time or job share position.

The post holder will be employed by Mowbray House Group on behalf of Hambleton North PCN but will be based across both Mowbray House Surgery and Mayford House Surgery serving both practice populations.

Main duties of the job

We are seeking to recruit an enthusiastic and experienced Gerontology Advanced Nurse Practitioner to join our existing and established frailty teams working across Northallerton.

About us

Mowbray House Surgery is a large progressive GP Surgery in Northallerton, we have a branch surgery at Hutton Rudby and between the two sites we look after around 20,000 patients.

Each of our sites have a dispensary on site and we dispense for nearly 8500 patients.

Job description

Job responsibilities

Duties and responsibilities:

  • Undertake visits to housebound patients as requested, assessing patients, liaising with the wider multidisciplinary clinical team where appropriate.
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for older patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over the counter medication regimens, side effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
  • Undertake frailty assessments on patients registered with the practice who have been identified as frail and support them with advanced care planning.
  • To maintain effective communication with multidisciplinary teams to the benefit of the patient and join the Practice's MDT meetings

Qualifications

Essential

  • Masters Degree Level in the relevant area of expertise;
  • Has either graduated from a Centre of Advancing Practice accredited
  • MSc Advanced Practice Programme or completed the Centre ePortfolio (supported) Route;

Experience

Essential

  • Experience of working in Secondary Care and/or a GP Practice
  • Experience of selecting and prescribing a wide range of medicines appropriately, including antibiotics and analgesia
  • Have a current valid UK Driving Licence and be a car owner with appropriate insurance

Desirable

  • Experience of SystmOne Clinical System
