Job summary

Clevedon Medical Centre are a GP Surgery based near Bristol looking for an enthusiastic Nurse with experience or an interest in care of the elderly.

Main duties of the job

Main Duties

Provide care and care management of patients with complex clinical needs and undifferentiated undiagnosed conditions. This will include history taking, clinical assessment, investigation, diagnosis, care planning and evaluation of outcomes to inform the on-going support and care management to be undertaken.

Chronic Disease Management Use EMIS searches to identify patients who need chronic disease reviews, and accurately complete Ardens templates for chronic disease management for Quality Outcomes Framework and Local Enhanced Services

Clevedon Medical Centre is based in a seaside town within easy commute of Bristol. We are a long-established teaching and research practice. Opposite the practice is sited the Community Hospital offering a full range of services and the practice looks after the inpatient beds.

A bit about the team:

We look after approximately 16,000+ patients and have a team of 5 Partners, 8 Salaried GPs and a full complement of non-GP clinicians

We work closely with other practices in our Primary Care Network. We aim to meet for coffee daily to catch up on, and discuss, clinical cases.

We are a friendly and forward-looking team committed to providing high quality primary care and we are looking to recruit a Health Care Professional with an interest in care of the elderly , starting as soon as possible.

Job description

Job responsibilities

The successful candidate will be expected to:

Review all new admissions to a care home.

Provide expert and timely advice to the homes including telephone triage and planned visits.

Must be confident in reviewing patient observations and conducting clinical assessment as needed. Would also need to provide advice and coordinate wound care with the district nurses.

To participate in the immunisation programme for older people.

Implement key learning from the national Partnership for Older People Projects (POPP) programme and the National Service Framework.

Work with the care homes to develop and implement best practice policies for the care and management.

Mental Health - Carry out basic assessment of patient for common mental health problems in the elderly such as dementia, depression and anxiety; and discuss with GPs as needed.

Develop strategies for the improvement of the daily living environment in homes to improve residents mental health.

End of Life Care Regularly discuss and update patients respect forms to ensure the patient wishes have been accurately been recorded

Provide regular ad hoc reports on activities highlighting achievements and areas of concern making recommendations for improved practice and provision.

Attend safeguarding meetings as and when required.

Medication Management In conjunction with the medical and pharmacy teams, review medication of patients with particular emphasis on the following

1. Medication Monitoring

2. Medications that might be implicated in increased risk of falls.

3. Analgesia and the use of NSAIDs

4. Psychotropic medication

5. The management of constipation.

6. Diuretic and heart failure medication

QOF/Local and Direct Enhanced Services

Chronic Disease Management Use EMIS searches to identify patients who need chronic disease reviews, and accurately complete Ardens templates for chronic disease management for Quality Outcomes Framework and Local Enhanced Services

Good understanding of QOF targets with for a review in a timely manner.

Complete relevant searches for patients with poor disease control and target them for additional support e.g., for weight loss/drug compliance.

Administration & General Duties

Regular management of emails

To review and action letters in relation to patients in a care home using our docman management software.

Action blood results as and when required.

Attend a minimum of 3 out of the 6 whole practice team meetings each year.

Support Covid and Flu clinics in the administration of vaccines.

Utilise and develop own knowledge of the Mental Capacity Act, safeguarding policies and guidance related to vulnerable adults at all times in the best interest of the patient.
