Job summary

An exciting opportunity for a forward thinking motivated individual to join the team of Care Coordinators working within the St Pauls Primary Care Network in the heart of Cheltenham, Gloucestershire.

The role will suit an enthusiastic candidate who is able to work collaboratively across the team and keen to deliver the ethos of the network. You will workwith both patients and Healthcare Professionals to ensure seamless, organised and effective care.

Main duties of the job

Our Care Coordinators work closely with our patients to help them improve access, health outcomes and engagement. Care Coordinators play an important role within a PCN (Primary Care Network) to proactively work with people, including the frail/elderly, to provide coordination and navigation of care and support across health and care services.

About us

The Network comprises four practices (Weston House Practice, St Catherines Surgery, St Georges Surgery, Royal Well surgery). The PCN benefits from operating at a single site, St Pauls Medical Centre with a branch surgery in Hatherley.

Job description

Job responsibilities

The following are the core responsibilities of the Care Co-ordinator. There may be, on occasion, a requirement to carry out other tasks; this will depend on factors such as workload and staffing levels:

To cover the reception desk in the St Pauls Clinic Hub when needed,welcoming, and directing patients appropriately and supporting the clinical staff on duty.

To invite patients in for Chronic Obstructive Pulmonary Disease/Asthma/Spirometry appointments with lists provided by the practices. Liaising with the respiratory champion and keeping track of patient follow ups.

Support Care Coordinator managers in learning disability recalls. Contact learning disability eligible patients via SMS, telephone, or letter, taking into consideration their preferred method of contact, inviting them for their annual health checks and updating the records accordingly in line with standard operating procedure.

Supporting the Social Prescribing Team with the PCN Frailty project, which entails calling patients to gather information and deciding whether they will benefit from an appointment with the Elderly Social Prescriber or PCN Frailty Nurse.

Performing admin duties including the collection of data, inputting information from the SystmOne Hub onto data collection spreadsheets.

Support updating the PCN Website and social media, posting regular updates on the PCN Facebook page to engage with patients, share job adverts and community project work.
