Job summary

St Pauls Primary Care Network presents an exciting opportunity for a part time experienced Clinical Pharmacist to work across the four practices that form the network, all housed in St Pauls Medical Centre in the heart of Cheltenham, Gloucestershire, in the South West (Weston House Practice, St Catherines Surgery, St Georges Surgery and The Royal Well Surgery).

The successful candidate will work across the four practices as part of the Pharmacy Team closely supported by the GPs, existing Clinical Pharmacists and the wider clinical team.

Main duties of the job

The role will provide a mix of face to face and virtual patient centred consultations and telephone support to patients and external agencies. You will be expected to use your clinical skills to carry out a variety of tasks including prescribing within your competency, medication reviews; reconcile medications at hospital discharge and clinical letters; oversee complex needs of house bound and care home patients and look to review and de-prescribe in polypharmacy.

Ideally suited to an Independent Prescriber with experience in primary care, you will be expected to support less experienced pharmacists and technicians and encouraged to manage suitable long term conditions and use your skills within the clinical multidisciplinary team to advise on medicine optimisation, safety and appropriate prescribing in line with local and national policy.

Job Purpose

  • To work across the four St Pauls practices within St Pauls Medical Centre, Cheltenham ensuring that medicines management processes within these practices are optimised to achieve highly effective, safe and patient centred prescribing.
  • To work closely with individual practice leads to tailor the delivery of network priorities and targets with a practice-based focus.
  • As an independent prescriber, to undertake face to face and telephone-based patient consultations, reviewing patients with potential medication issues and long term conditions. This will include de-prescribing within the context of polypharmacy and frailty.
  • To offer professional leadership and facilitate shared learning with other less experienced clinical practice pharmacists and pharmacy technicians working in associated local practices and networks to support the development of their role.

Responsibilities underpinning the role

To enrol or be enrolled in or have qualified from an accredited training pathway that equips the post holder to be able to practice and prescribe safely and effectively in a primary care setting and in order to deliver the key responsibilities of the role. Currently, the Clinical Pharmacist training pathway: Primary Care Pharmacy Education Pathway. (PCPEP)

To be an Independent Prescriber or close to completion and be able to undertake face to face consultations with patients.

To receive support and supervision from a Senior Clinical Pharmacist and GP lead to allow the post holder to develop and do the job safely and confidently.

To plan and organise the post holders own workload, including audit and project work, and training sessions for members of the medicines management team, practice team, community pharmacy team, community nurse team, patients, carers, etc.

To attend local, regional and national meetings of relevance some of which will be defined within the national PCPEP for this role supported by CPPE, Manchester University.

To develop and facilitate a good working relationship with community pharmacists, hospital pharmacists and other local providers of healthcare.

To become familiar with System1 GP system and supporting IT and to record personally generated information and maintain a database of information relating to the work done in the practice(s).

Personally generated information includes information and records relating to audit and clinical work undertaken by the post holder, reference notes relating to clinical/technical information, etc.

The database includes maintaining up-to-date, detailed records of all work done in the practices for which the post holder is accountable (done by the post holder or others) using appropriate coding.

To undertake any other duties commensurate with the post holders grade as agreed with the Network/ practice lead GPs or the post holders line manager, including mentorship of members of the Pharmacy Team. .

To maintain registration as a pharmacist with GPhC and comply with appropriate professional codes and standards.

As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process. Some of this will be supported through the PCPEP Pathway.
