Job summary
We are
looking for a dedicated and compassionate Practice Nurse to join our friendly
multidisciplinary team. In this role, you will deliver high quality, evidence based
nursing care while working both independently and collaboratively with
colleagues across primary, community, and secondary care.
You will be involved in a wide range of clinical areas, including chronic disease
management, immunisations, screening programmes, infection prevention, health
promotion, and well man/woman services. You will also support our population
health approach, helping to reduce health inequalities and improve access for
our patients.
As
an important member of the practice team, you will contribute to service
development, promote best practice, and participate in ongoing training and

Main duties of the job
- Deliver evidence based nursing care across a wide range of clinical areas, including long term condition management, immunisations, cervical screening, wound care, health promotion, and infection prevention and control.
- Undertake patient assessments, provide routine and urgent nursing care within scope of practice, and offer advice to patients and their families on managing their conditions.
- Support population health approaches by contributing to public health campaigns, targeting health inequalities, maintaining disease registers, and using call/recall systems effectively.
- Work collaboratively with the multidisciplinary team and external agencies to ensure coordinated, high quality patient care.
- Participate in clinical audits, quality improvement initiatives, research projects, and the development or review of practice policies, protocols, and procedures.
