Job summary

Pendle East Primary Care Network are looking for an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to work up to full time.

We are looking for friendly, conscientious team members to compliment our current multi-disciplinary team in our delivery of primary health care to our patient population.

Main duties of the job

Responsibilities include:

Develop, implement, and embed health and wellbeing programs tailored to community needs

Perform on-call sessions with the on-call GP to triage patients effectively

Manage acute and chronic conditions, providing expert advice and creating specialized treatment plans for chronic disease patients.

Identify and support at-risk patients, mitigating long-term health complications.

Deliver advanced care in line with clinical evidence, NICE guidelines, and NSF standards.

Request, collect, and interpret pathology results, ensuring comprehensive patient assessment and treatment.

Offer consultations, adjust care plans, and refer patients to secondary or specialist care as needed.

Recognise, assess, and signpost patients presenting with mental health concerns.

Act as an independent or supplementary prescriber in line with professional guidelines.

Maintain accurate clinical records, utilizing SNOMED CT codes effectively.

Support clinical emergencies and handle safeguarding matters per national and local policies.

Understand and manage cases involving substance abuse or addictive behaviours.

Deliver opportunistic health advice during patient interactions.

Provide care to housebound patients as required.

This role also involves contributing to practice and PCN targets (e.g., QOF), delegating tasks appropriately, and liaising with external services to ensure holistic patient support.

About us

We are excited to be recruiting an Advanced Nurse/Clinical Practitioner to be part of Pendle East Primary Care Network (PCN), which is a collaboration of 5 GP Practices within the Lancashire/Yorkshire border who collectively care for over47,500 patients.

The PCN member practices are located at 4 sites: 2 GP Practices within a large modern purpose-built building and 3 GP Practices a short drive away.

The PCN is determined to meet the challenges of modern primary care by working innovatively and growing our multi-disciplinary team to provide a holistic approach to our patient population.

The PCN team already includes; General Practitioners, Social Prescribing Link Workers, Health &Wellbeing Coach, Clinical Pharmacists, Pharmacy Technicians, Paramedic, Mental Health Practitioners, Physician Associates, Care Coordinators and GP Assistants.
