Job summary
To provide and maintain a high standard of nursing care to the practice population with focus in the delivery of evidence-based practice for patients presenting with long-term conditions and acute problems. This includes screening, assessment, treatment and education and taking an active role in supporting the practice to maximise achievement against local and national targets in respect of the GMS contract and Enhanced Services.

About us
We have 5 experienced doctors supported by an excellent clinical and administrative team including Pharmacists, Mental Health Nurses, Physiotherapists, Practice Nurses, Health Care Assistant, Phlebotomists, Administrative Staff, Receptionists and the Practice Manager.

We are an experienced practice committed to the teaching and training of medical students from Cardiff University, and also trainee pharmacists and practice nurses.

We are part of the South Easter Cluster [Collaborative] comprising of 8 practices. The surgery is located close to the city centre in the Roath area with a mixed demographic and easily accessible with public transport.
We hold regular informal clinical team meetings in addition to an open-door policy for mentoring and advice. We also have regular MDT meetings attended by our community teams including Midwives, Health Visitors, District Nurses and Palliative Care Nurses.
