Job summary

We are looking to appoint an experienced, passionate and committed Advanced Nurse Practitioner who has a teamwork approach to join our small team. The position available is for two days a week only on Mondays & Thursdays 8am-5:30pm.

Main duties of the job

  • To act as an autonomous practitioner working independently and in conjunction with other health care professionals
  • Assess, diagnose, treat and refer patients where appropriate.
  • Prioritise and triage patients presenting with a range of acute and chronic conditions
  • Implement and evaluate specialised treatment plans
  • Arrange and interpret pathology and other tests required
  • Maintain accurate clinical records
  • Ensure read codes are used effectively
  • Visit housebound patients

About us

We are a small friendly team consisting of a GP partner, salaried GP, ANP, Pharmacist, Practice Nurse and HCA's. We have a Practice Manager, Office Manager and 5 receptionists. In addition to our regular staff we also have Leigh PCN ARRS working from our building throughout the week. Our practice has alist size of just under 5,000 patients under the GMS contract. Based in a LIFT building in Leigh you will have access to modern facilites and parking.

Job description

Job responsibilities

  • To be able to deliver a high standard of care using autonomous clinical skills inadults and children
  • To be able to complete an episode of care for patients of all ages with a variety of health conditions.This would encompass high quality of care for patients who require urgent/ acute episodes/ long term conditions and provide health promotion and public health advice.
  • To use advanced practice skills within consultation to assess, diagnose, advise, treat and refer patients
  • To provide a comprehensive assessment
  • Arranging and interpreting appropriate investigations
  • Develop, implement and embed health promotion and well-being programmes
  • Support the clinical team in the development of individual treatment plans for chronic disease patients
  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
  • Ensure read codes are used effectively
  • Chaperone patients where necessary
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies
  • Deliver opportunistic health promotion where appropriate
  • Practice within the policies and guidelines of the NMC
  • Maintain confidentiality of all patients identifiable information and contacts
  • Maintain and develop clinical knowledge and competence using best practice, evidence, guidelines & supervision

  • Effectively manage workload, planning prioritising and co-ordinatingworkload
  • Work in partnership with other clinicians on duty to maintain effective communication

  • Undertake telephone triage for urgent on the day appointments, utilising effective communication skills and clinical judgement to triage patient safely or take the appropriate course of action either routine or urgent appointment.
  • Emergency home visits for urgent illness and chronic disease, to provide assessment of patients who may require a care plan,follow up to discharge from hospital and management of patients within practice aligned care home.
