Job summary

Were looking for an Advanced Care Practitioner (ACP) to join our welcoming and supportive team at Glastonbury Surgery for 37 hours per week.

This is an exciting opportunity to work within a supportive, patient-focused GP practice providing high-quality primary care to our local community.

As an ACP, youll work with a good level of autonomy while also being well supported by our GPs and nursing team. Youll see a varied mix of patients, manage both acute and long-term conditions, and play an important role in helping us deliver safe, effective, and compassionate care.

We welcome and encourage informal visits to the practice, giving you the opportunity to meet the team and learn more about the role before applying.

You will be supported with a comprehensive and tailored induction plan, with a commitment to ongoing training and development.

This role will be employed through West Mendip PCN (ARRS) and working full time at Glastonbury Surgery.

Main duties of the job

  • Assessing, diagnosing, and treating patients

  • Managing acute, chronic, and complex conditions

  • Prescribing independently (where qualified)

  • Referring patients to other services when needed

  • Working closely with GPs, nurses, and the wider team

  • Keeping clear and accurate clinical records

About You

  • A qualified Advanced Care Practitioner with relevant training

  • Registered with the NMC or HCPC

  • An Independent Prescriber (or working towards this)

  • Confident, approachable, and patient-focused

  • Enjoys working as part of a team and supporting service development

Job responsibilities

  • assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  • manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
  • use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
  • actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person; and
  • complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
  • undertake structured surgeries working in an autonomous role including the management of patients presenting with new problems or exacerbations of chronic disorders.
  • Engage in the planning and delivery of patient home visits, including both acute visits and the management of chronic medical problems in the house-bound.
  • Liaise with and advise the nurse lead to ensure most appropriate use of resources.
  • Be a training and mentoring resource for peers and hosted trainees.
  • Responsible for the collection and presentation of data that will lead to development of the service.
  • Promote the appointment system within the Practice, working as part of a same day service supported by a duty Doctor for guidance and consultation.
