Job summary

West Mendip PCN is a progressive Primary Care Network that is looking to recruit Paramedics to join us, supporting and working alongside a multi-disciplinary team.

Paramedics are authorised to undertake treatments and procedures as directed by the GP's for which they are trained in, and which are within the college of paramedic guidelines and in line with the HCPC standards framework.

The successful candidate(s) will be based* in a local cluster of General Practices as part of West Mendip Primary Care Network (WMPCN). They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.

*There are 2 positions available, one based in Wells and the other in Glastonbury. There is an expectation that the postholder(s) will work across the PCN area to provide cover for other PCN Practices if required.

Main duties of the job

To assess, diagnose, treat, refer or signpost patients/service users, who attend the surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.

The role will provide a specialist paramedic resource for healthcare professionals and service users, working in collaboration with other members of the multi disciplinary team.

To work within the surgery, as an autonomous, accountable paramedic, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment to deliver quality patient services.

The post holder will used advanced clinical skills to provide education to patients and service users, promoting selfcare and empower them to make informed choices about their treatment.

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the Practice or PCN team.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.

Be able to:

perform specialist health checks and reviews.

perform and interpret ECGs; alongside other results as appropriate.

perform investigatory procedures as required.

undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.

perform investigatory procedures needed by patients and those requested by GPs and the wider PCN.

Support the delivery of anticipatory care plans and lead certain community services (e.g., monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing)

Communicate at all levels across PCNs and other organizations, ensuring effective, patient-centred service.

Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.

Collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care.

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Prescribe, issue (if a NMP) and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

Work with patients in order to support compliance with and adherence to prescribed treatments.

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs.

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.

  • BSc (Hons), FdSc, DipHE in Paramedic Science or similar
  • Completion of an FCP taught module OR has received stage one roadmap sign off
  • Health & Care Professions Council (HCPC) registration
