Job summary

We are looking for an experienced ANP or ACP to join our team either short or long term.We are a forward-thinking practice with just over 7000 patients across our 2 sites in the Totley (S17) and Woodseats (S8) areas.

Main duties of the job

The ACP/APP/ANP will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment & prescribing.

About us

We pride ourselves on delivering high quality clinical care with a varied clinical team which consists of 2 GPs, 1 Physician Associate, a Practice Nurse, 2 Health Care Assistants plus a pharmacy team that includes 2 pharmacists and 3 Pharmacy Technicians all supported by our Administration team.

  • Be responsible for the assessment of health and clinical needs of patients, undertaking consultations as an autonomous practitioner, using their advanced clinical practice skills, and own clinical judgement to diagnose, treat, prescribe and refer.
  • Be proactive in clinical decision making, underpinned by an advanced level of theoretical and practical knowledge.
  • Be included in the Duty GP rota as required by practice needs and the clinical role.
  • Maintain a high standard of patient focused care in accordance with practices guidelines and policies.
  • Enable the clinical team to work effectively and provide a high quality service to patients and to work effectively with other agencies.
  • Use professional judgement in managing complex and unpredictable care events and capture any learning from these experiences to improve patient care and delivery.
