An exciting opportunity has arisen to join our forward-thinking, hardworking, training practice based in a beautiful rural location just outside Ashford in Kent. We are recruiting an enthusiastic team player to join our team of 3 Partners and 7 Salaried GPs for 4 -8 sessions per week.
We are a very supportive practice that holds daily catch up meetings, weekly clinical meetings and regular clinical education events.
We are a friendly and dynamic team with a full range of support staff including management, Advanced Paramedic Practitioner, nursing team, secretarial team, reception and administrative team, dispensary team, Clinical Pharmacist, Children's Health and Wellbeing Navigator, Cancer Care Coordinator, Social Prescriber and Mental Health Practitioner.
We hold regular clinics at the surgery including ENT, SIPC, MSK, Cardiology, Vasectomy, General Surgery and Ultrasound.
Main duties of the job
As a key member of the Wye Surgery team
you will manage a caseload dealing with a wide range of presenting
health care needs.
You will deliver excellent patient
care through continuous review of evidence-based clinical standards.
You will contribute to the quality,
monitoring and evaluation of the services provided by Wye Surgery and to the development of these services.

Clinical
- Make professional autonomous decisions in relation to presenting problems, whether self-referred or referred internally within the practice
- Continue to manage/follow up patients with conditions necessitating such and in a manner agreed with the partners
- Receive patients with undifferentiated and undiagnosed problems and make an assessment of their care needs.
- Screen patients for disease risk factors and early signs of illness.
- Develop with the patient a care plan for health
- Provide counselling and health education
- Admit or discharge patients to and from your caseload and refer to other care providers internally or externally as appropriate.
- Comply with relevant Practice policy/guidelines eg. In relation to child protection, confidentiality issues etc. as detailed in Practice policy and procedures
- Be committed to learning, audit and effectiveness issues to ensure evidence based best practice.
- Provide telephone advice service in accordance with practice protocols
- Provide opportunistic preventative/screening services
- Work in collaboration with other health care providers and agencies to ensure appropriate, co-ordinated services to patients and their families
Communication/Relationship
- The post-holder will report regularly to the Partners and ensure that they are suitably informed about changes in patients condition/well being, to maintain the high standards within the practice
- Foster good working relationships with other statutory and voluntary bodies
Education & Training
- Own Personal Development Plan as agreed with partners
Organisational
- Consult with patients in the surgery and at home visits as set out in an agreed schedule
- Be available for on-call duties on all working days although normally on call will only be one day per week. Times of on call will depend on the practices requirements.
- Contribute to the evaluation/audit and clinical standard setting with colleagues in the practice
- Record data in the patient record system to agreed standards with the practices clinical computer system and undergo any training where it is required
- Contribute to the strategic management and development of the practice eg. In developing and contributing to practice polices and guidelines.
