Practice Nurse (Salary negotiable based on experience and skillset) Hours: 37.5 Full Time (Job Share
will be considered)
East Durham Medical Group is seeking an
enthusiastic, forward-thinking experienced Practice Nurse to join our highly
committed primary health care team.
We are a friendly, teaching and training
SystmOne Practice with 18500 patients.
We have a fantastic opportunity for an
experienced Practice Nurse to join our multi-disciplinary team. We take pride
in delivering quality care to our patients and have a strong training and
personal development ethos. We are looking to recruit another member to our
team who shares the same vision and values.
We are a 6 day a week service and covering 6-8 pm and weekends will be required on a
rotating basis.
The ideal candidate will be based at 2 of our 6
sites, but would be expected to travel when needed to the other sites.

Main duties of the job
We are looking for an experienced Practice Nurse who has a passion for working in primary care and is able to quickly fit into working within a busy practice.
Desirable areas of experience would be:
Chronic disease management (Diabetes, Asthma, COPD etc.)
Cervical cytology
Immunisation and vaccinations
Contraception and sexual health
Wound Care
Travel Vaccs (Optional)

achieving, PMS training practice with a patient list of over 18 500 patients.

Job responsibilities
To be responsible for the delivery of nursing services, working
as part of the practice multidisciplinary team, delivering care within nurses
scope of practice to the entitled patient population. The general practice
nurse will be responsible for a number of clinical areas such as health
promotion, chronic disease management, health prevention as well as supporting
the management team in the reviewing of clinical policy and procedure.
We will

environment as well as experienced practice nurses.
Main duties of the job
The following are the core responsibilities of the General
Practice Nurse. There may be on occasion, a requirement to carry out other
tasks; this will be dependent upon factors such as workload and staffing
levels:
a.
Develop, implement and embed health promotion and well-being programmes
b.
Support the clinical team in the development of individual treatment plans for
chronic disease patients
c.
Identify, manage and support patients as risk of developing long-term
conditions, preventing adverse effects on the patients health
d.
Provide routine nursing care to patients as required in accordance with
clinical based evidence, NICE and the NSF
e.
Provide wound care (ulcer /Doppler etc.) to patients
f.
Provide travel medicine services
g. Refer
patients directly to other services as appropriate
h.
Provide chronic disease clinics, delivering patient care as necessary,
referring patients to secondary / specialist care as required
i.
Maintain accurate clinical records in conjunction with extant legislation
