Job summary

Are you an experienced and empathetic professional eager to make a meaningful impact in primary care? Our busy and friendly GP practice is seeking a dedicated Care Coordinator / Reception Manager to join our team and ensure the smooth operation of our front desk and patient care coordination.

This role is based across our main site at Bedford Street Surgery and our branch at Furzton Medical Centre. You will manage and support the reception and administrative teams, driving high-quality, patient-centered care. You will assist the management and clinical teams by monitoring premises and protocols, ensuring they are up to date.

As the key contact for patients, you will oversee the development and progress of care navigators with support from supervisors. You will also contribute to audits, projects, and play an essential role in practice and multidisciplinary team (MDT) meetings.

Main duties of the job

  • Manage and support the reception and administrative team to deliver high-quality, patient-focused service.

  • Coordinate reception staff schedules and rotas, ensuring adequate cover during leave and staff absences, working seamlessly with supervisors.

  • Oversee the coordination of patient appointments, follow-ups, and care navigation.

  • Monitor the day-to-day running of reception to ensure efficient workflow.

  • Serve as a key point of contact for patients, clinicians, and external partners.

  • Implement and monitor triage systems using Footfall and S1; undertake medical reporting and patient consent processes.

  • Support clinical and care coordination teams in achieving QOF, IIF, and other performance targets.

  • Ensure compliance with CQC and NHS policies and procedures.

  • Review and update reception procedures and pathways as required.

  • Attend meetings, preparing and producing minutes and agendas.

  • Support the management team with compiling practice reports and developing the practice plan.

  • Lead projects within designated timeframes to meet deadlines.

  • Conduct staff reviews, training, and appraisals.

About Our Practice

We operate from two sites: Bedford Street Surgery in Bletchley and Furzton Medical Centre, serving a growing patient population of over 16,000. Our practice is a partnership of five dedicated doctors committed to providing excellent primary care.

As our patient base continues to expand, so does our team. Across both sites, we employ a diverse range of healthcare professionals, including practice nurses, nurse practitioners, healthcare assistants, paramedics, pharmacists, and specialist nurses. Together, they form a multidisciplinary team (MDT) that meets monthly to discuss and coordinate patient care.

Our administration is supported by dedicated care coordinator and care navigator teams who manage healthcare appointments, follow-ups, and care signposting. These teams are further supported by secretarial staff, a finance team, and the practice manager, all working collaboratively to ensure seamless service delivery and high-quality patient care.
