H&F Partnership

Clinical Pharmacist - GP Surgery E-Hub FULL TIME or PART TIME

The closing date is 28 February 2026

Job summary

Hammersmith and Fulham Partnership PCN is offering a full-time or 2 x Part Time opportunity/ies for motivated and forward-thinking pharmacist/s to take on working within the H&F Partnership EHUB (e-Hub), delivering high-quality clinical pharmacy care to patients who access our GP practice through PATCHES, a leading online consultation platform used across London and the NHS. PATCHES allows patients to submit requests digitally at a time that suits them which creates a steady, clinically rich stream of work that is structured, prioritised and safe.

This role offers exceptional flexibility, with the potential for predominantly remote working. Many pharmacists find this enables better focus, fewer interruptions, and improved work-life balance while still being fully connected to a supportive clinical team. Attendance at practices is required for induction, training and team days, ensuring you remain embedded within the Partnership. You would be welcome to our GP sites whenever wishing or needing.

Main duties of the job

You will manage a wide range of clinical work, including long-term condition reviews, medication optimisation, contraception, HRT, and mental health queries such as anxiety and depression. This will all be from a secure digital workflow that supports good clinical decision-making, continuity and safety-netting.

As an Independent Prescriber, you will work with a high degree of autonomy. Triaging, consulting, prescribing and following up patients to resolution. You will be trusted to make clinical decisions, supported by clear pathways and immediate access to GPs and senior pharmacists when needed.

You will join a well-established multidisciplinary digital team of GPs, pharmacists and administrators who collectively manage patient requests efficiently and safely
