Job summary

We have a team of Care Coordinators already in post and are looking for another person to join the team.

Main duties of the job

This role involves:

Communicating with patients to ensure they understand their condition, care pathway and treatment and ensure that they are receiving the most appropriate support.

Support patients and practices to achieve various health checks and chronic disease reviews done in a timely manner. You will be co-ordinating booking and following up DNAs, etc.

Working very closely with the practices, care homes, multidisciplinary teams within the PCN. The role is pivotal in ensuring our patients receive the best possible care and service.

The Health Triangle Primary Care Network have an exciting opportunity for a Care Co-ordinator to join our Team, supporting our 3 GP practices (Ringmead, Evergreen and Crownwood Medical Practices) to assist in re-shaping Primary Care across our community. This post is mainly to cover Ringmead patients at present.

Job description

Job responsibilities

Key responsibilities:

1. Undertake work in line with PCN directed priorities.

2. Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids

3. Ensure regular and consistent communication with the referrer regarding patient progress and any complications or guidance

4. Raise awareness of health promotion and NHS health checks in practices

5. Lead of the Safe recruitment and induction of all seconded staff into the PCN.

6. Evaluate treatment programmes that promote health and well-being

7. Co-ordinate clinics, liaising with the schedulers and contacting the identified patients with appointments.

8. Manage patient initiated calls for help/signposting etc, booking into named GP urgent care slots/telephone slots if necessary.

9. Document and monitor aspects of patient co-ordination and service delivery, supporting data collection and audit using the patient administration system

10. Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or emergency situation, alerting the team or enabling a rapid response.

11. Dependant on work plans, there may be a requirement to work across different member practices and teams.
