Job summary

We are pleased to be able to offer an opportunity in becoming part of a dynamic and thriving Multi-Disciplinary Team. The successful candidate will be part of the on the day service team, which will include telephone triage, face to face consultations and, as necessary, visiting patients in their home. We are fortunate in having a plethora of experience and skills within the team, allowing individuals to flourish further and achieve personal satisfaction. We provide daily mentorship and hold educational sessions several times per month. The successful candidate will join the team in continuing to provide a high standard of clinical care and creating a service that effectively and efficiently meet the needs of our patients.

Main duties of the job

  • Assist in telephone triage of 'on the day' patient requests, advising or signposting for self-care, arranging face-to-face consultations or arranging a home visit
  • Prioritise presenting patients' health problems appropriately in urgent or emergency situations
  • Provide information and advice to patients in relation to prescribed or over the counter medications, side effects and interactions
  • Support patients to adopt health promotion strategies
  • Assess, diagnose, plan, implement and evaluate treatment/ interventions and care for patients presenting with an undifferentiated diagnosis on an acute basis
  • Clinically examine and assess patient needs from a physiological and psychological perspective
  • Arrange investigations with practice nursing team as appropriate for patients attending 'on the day' service
  • Arrange investigations with secondary care or community services
  • Follow up patients in the short term ensuring continuity of care
  • Discuss with the duty doctor the needs of patients presenting with complex health needs
  • Liaise with secondary care and community teams as appropriate
  • Work alongside Minor Illness Triage Nurse in relation to clinical assessments and treatment
  • Undertake home visits as assessed by the duty doctor
  • Participate in the delivery of the Quality and Outcomes Framework (QOF) and Investment and Impact Fund (IIF)

We also encourage the MDT to pursue special interests beyond minor illness. With one of the practices as a training practice, our Paramedic Practitioners have gone on to develop specialist skills in musculoskeletal medicine, palliative care, dermatology, oncology, and long-term condition reviews. Training is always supported amongst the GPs and MDT.

Main Duties and Responsibilities

Clinical

  • Interpret test results such as ECGs, urinalysis, basic laboratory reports and plan clinical care accordingly
  • To liaise with the practice nurse team for appropriate follow up for patients who require wound management or phlebotomy.
  • To arrange investigations as appropriate with secondary care or community services when a need is presented
  • To follow up patients in the short-term to ensure continuity of care
  • Work collaboratively with other health care professionals and disciplines.
  • Provide a supportive role in helping people to manage and live with illness.
  • To discuss with the supervising doctor the needs of patients presenting with complex health needs or conditions requiring treatment, hospital admission or referral
  • Undertake home visits within the boundaries of professional qualifications and in accordance with the practice Home Visit Policy
  • To ensure that any information or advice provided is evidence based and that knowledge and skills are maintained for safe and effective practice
  • Participate in the delivery of the Quality and Outcomes Framework (QOF) and support the practice with clinical trials including audit

Administrative responsibilities include:

  • Participate in the administrative and professional responsibilities of the Practice team.
  • Ensure accurate and legible documentation of all consultations and treatments are recorded in the patients notes.
  • Use of IT systems such as Accurx to communicate with patients
  • Attend and participate in meetings and peer support / learning as required
  • Restocking and maintenance of clinical areas and consulting rooms. Supplies and equipment
  • Maintenance of equipment and stock relating to patient care
  • To help cover staff holidays and sickness when required

Training and personal development

  • Develop and maintain a Professional Portfolio using national guidance such as Health Education Englands Specialist / Advanced Level Practice guidance as structure.
  • Maintain requirements for ongoing HPC registration and, if available in the future, national registration with any other Specialist / Advanced level directory of staff (HEE).
  • Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the PCN.
  • Participate in the education and training of students of all disciplines and the introduction of all members of the Practice staff where appropriate
  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
  • If it is necessary to expand the role to include additional responsibilities, full training will be given.
