SALARY: £35,100.00 - £46,800.00 per year

The Health Triangle PCN is a flourishing organisation, comprising a diverse Multi-Disciplinary Team including Doctors, Nurses, Advanced Practitioners, Paramedic Practitioners, Clinical Pharmacists, First Contact Physiotherapists, Nursing Associates, Mental Health Practitioner, Social Prescriber and Care Coordinators.

The PCN consists of 3 practices serving 50,000 patients spanning over Bracknell/Crowthorne and Sandhurst. We are a friendly and innovative organisation, with a strong focus on education and personal development.

We are delighted in being able to offer a Paramedic Practitioner position to join our experienced and enthusiastic clinical team.

The PCN strives to develop the MDT with support from the local training Hub, and with a training practice within the PCN, there is an embedded ethos of support and growth. We have a learning structure in place to train paramedics on the FCP and ACP pathway including advanced assessment and prescribing. We also encourage the MDT to pursue special interests beyond minor illness. With one of the practices as a training practice, our Paramedic Practitioners have gone on to develop specialist skills in musculoskeletal medicine, palliative care, dermatology, oncology, and long-term condition reviews. Training is always supported amongst the GPs and MDT.

We are also flexible with our session times and aim for a good work-life balance (Part-time will be considered)

There is also the opportunity to work in enhanced access clinics which currently offers an additional payment incentive.

Main Duties and Responsibilities

Clinical

• Telephone consultations of on the day patient requests, to advise on or signpost for self-care, arrange for face-to-face consultations to either see themselves, another member of the clinical team, the Nurse Practitioner, a Practice Nurse/HCA, or to arrange a home visit
• To prioritise presenting patients health problems appropriately in urgent or emergency situations, including initiation of emergency care if required
• Assess, examine, diagnose, plan, implement and evaluate holistic treatment/interventions and care for patients presenting with an undifferentiated diagnosis on an acute or minor illness basis
• Provide information and advice to patients in relation to prescribed or over the counter medications, side effects and interactions.
• Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care where appropriate.
• Interpret test results such as ECGs, urinalysis, basic laboratory reports and plan clinical care accordingly
• To liaise with the practice nurse team for appropriate follow up for patients who require wound management or phlebotomy.
• To arrange investigations as appropriate with secondary care or community services when a need is presented
• To follow up patients in the short-term to ensure continuity of care
• Work collaboratively with other health care professionals and disciplines.
• Provide a supportive role in helping people to manage and live with illness.
• To discuss with the supervising doctor the needs of patients presenting with complex health needs or conditions requiring treatment, hospital admission or referral
• Undertake home visits within the boundaries of professional qualifications and in accordance with the practice Home Visit Policy
• To ensure that any information or advice provided is evidence based and that knowledge and skills are maintained for safe and effective practice
• Participate in the delivery of the Quality and Outcomes Framework (QOF) and support the practice with clinical trials including audit

Administrative responsibilities include:

• Participate in the administrative and professional responsibilities of the Practice team.
• Ensure accurate and legible documentation of all consultations and treatments are recorded in the patients notes.
• Use of IT systems such as Accurx to communicate with patients
• Attend and participate in meetings and peer support / learning as required
• Restocking and maintenance of clinical areas and consulting rooms. Supplies and equipment
• Maintenance of equipment and stock relating to patient care
• To help cover staff holidays and sickness when required

Training and personal development

• Develop and maintain a Professional Portfolio using national guidance such as Health Education Englands Specialist / Advanced Level Practice guidance as structure.
• Maintain requirements for ongoing HPC registration and, if available in the future, national registration with any other Specialist / Advanced level directory of staff (HEE).
• Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the PCN.
• Participate in the education and training of students of all disciplines and the introduction of all members of the Practice staff where appropriate
• Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
• If it is necessary to expand the role to include additional responsibilities, full training will be given.

Experience

Essential

• Post-registration experience in a substantive pre-hospital,
• Emergency Department or Primary Care role with evidence of credible experience to the role specification
• Experience of research and audit
• Experience of complex case management
• Expert knowledge of pre-hospital care and urgent care systems
• Clinical examination skills
• Excellent communication skills including written and verbal; good telephone skills
• Experience dealing with difficult patients / conflict management Effective record keeping
• Able to assess risk
• Able to work within professional boundaries and qualifications

Desirable

• Experience of working in a telephone triage role
• National and local priorities
• Knowledge of public health issues
• Primary Care experience Experience in the use of EMIS computer system Experience in training others

Qualifications

Essential

• Registered Paramedic with Health Care Professions Council or Nurse with the Nursing and Midwifery Council (UK)
• DipHE/FDg in Paramedic Sciences or equivalent healthcare qualification Level 5 or 6
• HEI module in Minor Injury/Illness or equivalent Level 5 or 6
• HEI module in mentoring or equivalent

Desirable

• BSc in Paramedic Sciences or other relevant healthcare qualification (or working towards)
• MSc in Paramedic Sciences or other relevant healthcare qualification (or working towards) Level 5 or 6
• HEI module in telephone triage
• Independent Prescriber qualification or willing to work towards

Other

Essential

• Willingness to work flexibly to cover holidays and sickness, including evenings
• Full clean driving licence, access to car with current insurance Willingness to participate in staff training and meetings
• Understanding of Confidentiality and Data Protection, Equality and Diversity and Safeguarding
