Job summary

We are seeking an Enhanced Practice Nurse (EPN) who will work as part of our clinical assessment team. The role will include working within our clinical allocation hub as well as supporting and working within our home visiting team and developing our urgent care/minor illness service.

We are looking for a highly motivated, enthusiastic individual with appropriate qualifications and clinical skills in general practice or urgent care.

Our ideal candidate would have experience in a fast-paced environment, be confident in their clinical decision making and be able to deliver a high standard of safe clinical care.

Our integrated hub is staffed by a team of GPs and non-clinical allocators who manage all patient requests. Our home visiting service is run in partnership with our community trust, and we have appointed to the position of Community Practitioner. We would expect the post holder to work alongside the Community Practitioner in delivering these functions.

The EPN would be a crucial part of our clinical assessment team, undertaking face-to-face consultations in urgent care/minor illness clinics, and in patients own homes.

Job summary

The Enhanced Practice Nurse is a key member of the Middlewood Partnership Primary Care Network multidisciplinary team. The post holder, acting within their scope of practice, will

Work within our clinical allocation hub to triage patient enquiries and requests.

Guide, signpost and move patients through our system to ensure they see the right professional in a timely manner.

Offer expert advice via telephone and face to face assessment on a range of minor illness and minor ailments, including more complex case management, as appropriate, within scope of practice.

Undertake home visits and clinical assessments as part of our visiting service. This will be with support and guidance from our visiting GP and working as part of an integrated multidisciplinary team.

Specific functions will be to

Work as part of the multi-disciplinary team in providing quality, evidence-based health care to meet the immediate needs of the patient.

Provide safe and effective online and telephone advice, face to face health assessments and advice and provide health and well-being information and advice to Middlewood patients.

Ensure all online and telephone assessments are of a standard to ensure the most appropriate clinical outcome for the patient is reached.

Take an active role within the multi-disciplinary team to ensure service quality in line with our organisational goals and strategy

Assist in the development of new models for service delivery which have a positive impact on the health care of our patients.

Carry out visits to patients homes, as well as visits to patients in Nursing/Care homes.

Provide an alternative model to urgent and same day GP home visits for the network.

Undertake clinical audits where required

Communicate at all levels across organisations ensuring that an effective, patient-centered service is delivered.

Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Be able to recognise, record and report adverse incidents to promote patient safety and effective organisational learning

Analyse reports in order to support and enhance patient care

Give self-care and minor ailment advice as appropriate, referring to other service providers where necessary, according to clinical need.

Deliver appropriate face-to-face assessments and clinical care, through history-taking, examination, investigation and treatment.

Keep accurate and immediate records of every assessment and advice given regarding patient care, using our clinical system.

Encourage patients to carry out self-care as appropriate through the effective communication of evidence-based health care advice.

Provide appropriate education to patients in the correct use of primary care and emergency care services and other health care related organisations.

Maintain patient confidentiality.

Ensure cases audited meet clinical and operational indicators to ensure continuous improvement of a quality service, in line with the clinical governance agenda.

To promote effective communication and networking within the team.

To participate in the evaluation of the service that is offered and implement changes as required.

Maintain professional registration & revalidation requirements of governing body affiliated to your professional qualification

  • Evidence of recent and ongoing study and self-development
  • Recognised qualifications and autonomous clinical assessment skills
  • Post basic education
  • MSc Clinical Examination /Minor Illness

Experience

  • Recent experience in acute / primary care setting.
  • Experience of undertaking online and telephone triage/assessment
  • Experience of working autonomously in patient contact services and / or primary care nursing
