Bay Medical Group recruiting for an Advanced Practitioner Clinical Lead to become part of our forward-thinking team serving over 54,000 patients in the vibrant Morecambe Bay area and shaping the future of primary care.
This is a permanent position, working between 28 - 37.5 hours per week. Working pattern must be worked over 4 days as a minimum if part time or 5 days if full time and each shift will be 7.5 hours each day. There is a requirement to work until 8pm one evening a week.
The post holder may be based at one of our site but will be expected to work across our 5 sites.
Interviews scheduled for week commencing 10th November 2025.
Main duties of the job
The AP clinical lead will be responsible for the development and integration of designated roles in the Practice team for the delivery of person-centred care for patients in Bay Medical Group. The post holder will be integral to the delivery of safe, effective, consistent and high-quality care for patients, and in collaboration with colleagues will be instrumental in promoting clinical excellence and best practice.
They will contribute to the design, implementation and evaluation of clinical activities, operating at a strategic level. They will aim to continuously improve standards of patient care, advance the boundaries of professional clinical practice and facilitate multi-professional approaches to service development and delivery.

The balance of these functions may vary from time to time in response to the needs of the Practice. The postholder will

Provide the first point of contact for clinical queries from the Patient Advisors on the CAT or Visit Team, providing specialist advice and support.
Deliver care through various consultation types, including in person clinic appointments, telephone consultations, home visits and e-consultations.
Practise in compliance with respective code of professional conduct and within own scope of practice, being responsible and accountable for decisions and actions.
An advanced level of clinical reasoning and decision making across a spectrum of practice.
Clinically examine, assess, diagnose, plan implement and evaluate treatment/ interventions and care for patients, using expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations; using information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.
Diagnose and manage both acute and chronic conditions, initiating, evaluating and modifying a range of interventions which may include prescribing and reviewing medicines for therapeutic effectiveness (appropriate to patient need and in accordance with evidence based practice and national protocols within scope of practice), therapies, life style advice and care into a management plan.

Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
Collaborate with other members of the Primary Health Care Team, accepting referrals and referring to them.
Refer patients directly to other services and agencies as appropriate.
Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families and carers.
