Job summary

Bay Medical Group are recruiting for a permanent part time Medicine Coordinator to work as part of the Medicine Management Team.

This role enables the Practice to offer quality patient care, by efficiently and safely dealing with queries regarding medication and directly supporting Bay Medical Group Pharmacist Team to deliver quality implementation and outcomes measures.

This is a part time vacancy working 27 hours per week. Working pattern for the role is Monday 8:30-17:30, Tuesday 8-13:00, Wednesday 13:00-18:00, Friday 8:30-17:30.

We are looking for an enthusiastic administrator who has excellent organisational skills, communication skills and knowledge of medication and medical terminology.

Interviews will be on the 21st July 2025.

Job responsibilities

MAIN RESPONSIBILITIES:

Management of Prescribing:

  • Ensure that all requests for repeat prescriptions are actioned, where possible, within two working days.
  • Check that all repeat items requested are due to be issued.
  • Add new medication appropriately in line with protocols
  • Add correct dose instructions to medication and change quantities accordingly.
  • Liaise with local pharmacists re medication queries.
  • Investigate and where appropriate resolve any patient non-compliance (requests for drugs that are unusually early or show that an item is not being used regularly).
  • Investigate & resolve any requests for non-repeated medication.
  • Update any drug data in patient computer records.
  • Ensure information from hospital discharge/ outpatient letters is input in computer records.
  • Communicate with or task Patient Advisors/ District Nurses/ Long Term Conditions team regarding any requests from secondary care for follow-up interventions.
  • Liaise with consultants, secretaries, Crisis Team, CPNs, Social Services and other Health care workers as required
  • Monitor the discontinuation of medications & make appropriate recommended changes (search for patients, alter medication, inform patients).
  • Rationalise patients medication e.g. initiate generic savings.
  • Able to allocate and re-allocate workload, tasks and staff if required to meet service requirements.
  • To work accurately and speedily to ensure deadlines.
  • To support service improvement programmes and audit/ monitoring of these service improvements to improve patient safety.
  • To be able to work flexibly to cover holidays and sickness absences.
  • Facilitate integration of all staff e.g. show new Patient Advisors/ admin team members the role of the MMT.
  • To complete all tasks of the Medicines Management Officers when required.

  • Support the practice in the delivery of the ICBs strategic medicines management plan.
  • To work with Primary Care Pharmacists on projects relating to the Medicines Optimisation LES and Quality & Outcomes Framework (QOF).
  • To undertake practice-based ICB project work under the direction of the Medicines Co-ordinators Scheme Lead, to ensure cost savings on the prescribing budget.
  • To help the Medicines Optimisation Team with any searches, data collection and letter production required for prescribing changes.
  • Identify patients who would be suitable for electronic repeat dispensing and Medicines Use Reviews (MURs).
  • Promote better use of medicines by developing and improving patient information.
  • Attend relevant courses & training events.
  • Effective use of EMIS to monitor specific medication.

Communication

The post holder will:

be empathetic to each patients situation to ensure effective communication and to prevent conflict.

  • share and disseminate information within the rules of confidentiality/ GDPR

apply skills and techniques (verbal and non-verbal) to resolve conflict and anger

deal with telephone enquiries and using their knowledge and experience to answer queries where possible or pass it on to a pharmacist/ clinician.

Training and development

The postholder will:

take an active part in learning/ development, attending those courses and seminars, which are deemed necessary by the manager.

identify development needs for their own learning and share these with manager

provide help and guidance to others (e.g. MMO) to support their development or to help them complete their work requirements effectively
