Job responsibilities
Personalised Care and Care Co-ordination
- Proactively identify and work with people who would benefit from care co-ordination, including those with complex needs.
- Support patients to develop and review their personalised care and support plans.
- Act as a central point of contact, helping patients navigate health, social care and community services.
- Liaise closely with GPs, nurses, social prescribers, health and wellbeing coaches and other MDT members.
- Assist with follow up after hospital care, transitions between services, and ongoing communication.
Administration and Data Quality
- Provide a welcoming, professional service to all patients and their carers.
- Book, amend and manage appointments (telephone, face to face and online).
- Book and manage home visit requests for the Advanced Nurse Practitioner.
- Support the Advanced Nurse Practitioner with administration related to role, including obtaining information prior to a first home visit to assessment support risk assessment.
- Process clinical messages and signposting queries.
- Handle incoming calls, emails, online requests.
- Support patients with test result queries and specimen handling in line with protocols.
- Update patient information where necessary.
- Maintain high quality record keeping and support data collection for audits/service improvement.
- Support the delivery of MDT meetings in the practice by facilitating health care professionals to attend. Attend MDT meetings to support any care co-ordination activities required during and following the meeting.
- Take minutes of MDT meetings and/or update records at the request of clinical staff.
- Work with the practice team to identify opportunities to develop the role within the practice setting.
- Administrative duties to support the management of the nursing team.
- Any other duties considered necessary by the doctors, nurses or the Practice Manager
Administrative and professional responsibilities
- Participate in the administrative and professional responsibilities of the practice team
- Ensure accurate notes of all consultations and treatments are recorded in the patients record on the clinical computer system, if requested to do so.
- Participate in audits when required.
- Attend and participate in any practice meetings as requested by the Practice Manager
- Participate in the education and training of students of all disciplines as required
- Attend and participate in any training deemed necessary as requested by the Practice Manager

In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. In the performance of the duties outlined in this Job Description, the post holder may have access to confidential information relating to parents and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from whatever source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
Staff are expected to report any problem that they encounter immediately to the Doctor on Duty.
