JOB TITLE : Practice
Nurse
REPORTS TO:
PRACTICE MANAGER & GP PARTNERS
HOURS: approx 32hrs
Job Summary
Manage the
day to day operations of the Practice in accordance with the Partners
instructions and to ensure Practice compliance with Contractual duties and
responsibilities including GMS Contract, Quality and Outcomes Framework and
Enhanced Services Schemes.
Treat,
assist and direct patients in accessing the appropriate service or healthcare
professional in a courteous, efficient and effective way. Provide general
assistance to the practice team and project a positive and friendly image to
patients and other visitors, either in person or via the telephone. Duties may
vary at times under the direction of the Practice Manager, dependent on current
practice workload and staffing levels. The post-holder will apply practice
policies, standards and guidance. Participate in audit where appropriate.
Job responsibilities
Administration
To be involved in the delivery of managerial and
administrative aspects of the Quality and Outcome Framework (QOF) including the
preparation and submission of documents and evidence required for annual QOF
review purposes.
Ensure practice compliance with CQC
requirements.
To have a thorough knowledge of all Practice
procedures.
To work in accordance of written protocols.
Prioritise workload to maximize efficiency
Practice Nurse duties
To provide all Practice Nurse duties, including
Immunisations, vaccinations, cervical smears, specialist clinics and general
every day practice nurse procedures.
To be involved with the general organisation of
the Practice ensuring that systems and procedures are in place to deliver high
quality, responsive primary care services to patients registered with the
practice.
To be involved with and implement policies and
procedures ensuring coherence to nation standards and legislation.
Communicating with patients via telephone or
other as would be expected at a GP practice.
Support Partners and other clinical staff in the
implementation and achievement of QOF clinical targets including regular review
of Population Manager.
To be involved in and ensure that the Practice
is compliant with the requirements of the General Medical Services (GMS)
contract and help ensure the practice meets the requirements of the Performance
Management Procedures.
Manage registers and implement recall systems
for patients with Long Term Conditions.
Consulting with members of the practice team and
outside agencies.
Ordering supplies, Vaccines & medications.
Computer
Use of Emis Web software system.
Make adjustments to electronic patient records
& update manual paper records as necessary.
Make referrals for patients to other agencies as
required
Confidentiality
Ensure that you adhere to strict confidentiality
guidelines at all times.
Be aware of anyone who may overhear your
conversation.
Ensure that any paperwork or onscreen
information cannot be viewed by a patient or anyone not employed by the
practice.
Information relating to patients, carers,
colleagues, other healthcare workers or the business of the practice may only
be divulged to authorised persons in accordance with the practice policies and
procedures relating to confidentiality and the protection of personal &
sensitive data.
Health & Safety
The post-holder will assist in promoting and
maintaining their own and others` health, safety and security as defined in the
practice Health & Safety Policy, to include:-
Identifying the risks involved in work
activities and undertaking such activities in a way that manages those risks.
Making effective use of training to update
knowledge and skills.
Using appropriate infection control procedures,
maintaining work areas in a tidy and safe way and free from hazards.
Reporting potential risks identified.

The post-holder will participate in any training
programme implemented by the practice as part of this employment.

and risk.
