Job summary

This is an exciting and challenging opportunity for practitioners at an advanced level to work within the Home Visiting Service for a collaborative of primary care practices across North & West Wrexham.

This is an established service where practitionersprovide a high-level of clinical expertise, knowledge and skills to patients within their own home or in a community setting. It will include taking the lead role in the clinical assessment, diagnosis, treatment and management of patients who may present with acute/urgent needs. The UCP will be expected to work closely within a multi-disciplinary team as well as autonomously at times.

Please note that this is a short-term contract to 31st March 2024. Continued funding past this is currently being evaluated by the local health board.

Main duties of the job

The post-holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.

The Urgent Care Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.

The post-holder will use skills, knowledge and competencies as a qualified Urgent Care Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.

About us

This role is within the North & West Wrexham collaborative of GP practices as part of the Home Visiting Service. It is funded by GP collaborative monies provided by the Welsh Government. The collaborative comprises:

  • Alyn Family Doctors in Llay, Gresford and Rossett
  • Caritas Surgery, Coedpoeth
  • Bryn Darland Surgery, Coedpoeth
  • The Health Centre, Coedpoeth
  • Pen y Maes Health Centre, Summerhill

The lead employer for administration purposes is Alyn Family Doctors, which will be responsible for contracting, payments and line management. However, the job role involves working across the collaborative according to an agreed job plan. The post-holder will report to named contacts in each practice for any matters relating to clinical care of patients of an individual practice.

JOB RESPONSIBILITIES

Clinical Practice

- Work as part of a multi-disciplinary team to deliver safe and high quality care.

- Carry out clinical urgent care practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.

- Delivery of care in line with NSF, NICE guidelines and evidence-based care

- Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.

- Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.

- Review medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.

- Request diagnostic investigations within agreed protocols e.g. X-ray, blood tests, scans, etc.

- Assess, diagnose and treat patients in practice premises or their own home who require urgent medical attention for treatment and/or refer to appropriate agency.

- Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.

Care Management

- Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition.

- Monitor and meet care outcomes against standards and recommend or initiate changes as necessary.

- Undertake investigations as appropriate.

- Liaise with other teams regarding patients care

- Be accountable for the safe clinical, practice applied to a patient which in your care

- Work collaboratively with other health care professionals and disciplines which will include communicating effectively with team members to ensure a safe and effective handover when required.
