Job summary
As a Long-term conditions nurse, you will provide and
maintain a high standard of long-term disease management, which includes
diagnosis, monitoring & development of individual management plans,
agreeing these as appropriate with the patient & other health
professionals.
Main duties of the job
Youll be involved in the smooth day to day running of the Long-Term
Condition Clinics.

professional boundaries, the post holder will deliver primary health care

delivery of policy and procedures, and demonstrate effective professional nurse
standards, advocacy, and performance.
