Job summary

As a Long-term conditions nurse, you will provide and maintain a high standard of long-term disease management, which includes diagnosis, monitoring & development of individual management plans, agreeing these as appropriate with the patient & other health professionals.

Main duties of the job

Youll be involved in the smooth day to day running of the Long-Term Condition Clinics.

Duties may include but are not limited to:

  • Experience of managing/dealing with the public/patients
  • Experience of working within healthcare governance systems and understanding role and responsibilities for CQC and within risk management
  • Experience of using clinical systems & Microsoft Office software

  • Experience in Primary or Community Care Nursing or equivalent relevant experience
  • Experience of audit and/or research

  • Experience of group teaching and 1:1 teaching to support patients with
