Job summary

As a Social Prescribing Link Worker within St Neots PCN, you will hold a vital role in empowering patients to take charge of their health and wellbeing. Social Prescribing Link Workers support and guide patients with diverse needswhether they are living with long-term conditions, battling low mood or loneliness, or facing complex social challenges. Working within our multidisciplinary team, you will assist individuals in accessing non-medical support, building their confidence, and connecting them with community resources that address the broader factors influencing their health and wellbeing

Main duties of the job

As a Social Prescribing Link Worker, you will support patients to improve their health and wellbeing by connecting them with non-clinical services and community resources. Working closely with GPs, the wider practice team, and local community services, you will provide tailored one-to-one support, empower people to take greater control of their lives, and contribute to reducing health inequalities.

Key responsibilities:

  • Provide personalised, one-to-one support using a what matters to me approach, co-producing care plans with individuals, families, and carers.
  • Empower patients to build confidence, self-management skills, and sustainable support networks.
  • Connect patients with voluntary, community, and statutory services to address wider social determinants of health.
  • Build and maintain strong relationships with local organisations and partners, supporting the sustainability of community groups and helping to address gaps in provision.
  • Work collaboratively with GPs, practice staff, health coaches, care coordinators, and other link workers to deliver integrated, personalised care.
  • Maintain accurate patient records, monitor outcomes, and provide feedback to support service improvement.
  • Ensure safeguarding, governance, and lone working policies are followed, and engage in supervision, training, and professional development.

The postholder will need to possess good computer skills to use local IT systems to support caseload working.

Lakeside Healthcare are looking to employ Community Link Workers (Social Prescribing) to assist in the provision of care for patients within our Primary Care Network (PCN).

The PCNs vision is to work together to deliver the best possible outcomes for our patients and staff. You will work in close partnership with our member practice teams as well as health, statutory and voluntary groups to create effective help and support for those that need it whilst being a key point of contact to enable patients to be supported within their local community.
