Job summary
South Peterborough PCNs Personalised
Care Team consists of Social Prescribing Link Workers, Health and Wellbeing
Coaches and Care Coordinators. They are a forward thinking and innovative team
who deliver personalised care to a high standard. The post holder will be responsible for leading
and supporting the team in their day-to-day work, undertaking case load
reviews, identifying training and development needs and being a general first
point of contact for any questions, queries and ideas. In addition to line
managing the team, the post holder will also hold a caseload of patients in
their area of expertise as Social Prescribing Link Worker; Health &
Wellbeing Coach or Care Coordinator.
Main duties of the job
- Supervision and daily line management of Personalised
Care Team members; being a first point of contact
- Support
the Digital and Transformation Manager with projects across the PCN involving
the Personalised Care Team
- Developing induction plans for new members of the
team; identifying training requirements and opportunities for the existing team
- Provide support and guidance through facilitating
regular peer review sessions
- Working with
practices to establish and maintain appropriate referral processes
- Supporting patients through the personalised care
model, focusing on strengths and needs of individuals.
- Support
the Digital and Transformation Manager with projects across the PCN involving
the Personalised Care Team.

supported by a dynamic PCN Clinical Director and an impassioned Health
Inequalities Lead, the PCN has undertaken several initiatives locally in the
past 2 years to support patients with managing complex pain conditions, weight
management and addiction services to great effect.
