Job summary
The role of the Community Nurse Practitioner is to work with the lead GP
in developing and providing a proactive and responsive community based primary
care service for patients based in the community with acute minor illness,
long-term conditions and complex case management of patients who are prone to
hospital admission. Some key areas of the role are to develop and review
admission avoidance care plans as well as contributing to and working with the practice
team and Specialist Neighbourhood Nurse; in actively managing the patients
health.
Main duties of the job
- Reviewing and providing care for patients in the community, whether for preplanned services or sameday urgent care.
- Diagnosing and treating patients presenting with minor illness.
- Managing ongoing medical and nursing needs of patients, including advice and followup care.
- Prioritising patient needs appropriately, including arranging investigations and making referrals when necessary.
- Any patient assessed as requiring secondary care referral should be discussed with the registered GP before the referral is made.

- RGN Qualification (Adult Branch or relevant)
- NMC 1st level Registration
- Evidence of continued professional development.
Desirable
- Certificate/diploma in community nursing Mentorship course
- Clinical Supervision experience/trainer
- Leadership course
- Accredited training in chronic disease management
- Non-Medical independent prescriber
Experience
Essential
- Evidence of continued professional development.
- Experience of working at level 6; ideally in community nursing or with transferable skills. Working autonomously in an independent role as well as in a team.
- Experience of providing emergency care for patients and management of patients with complex needs
- Evidence of wide knowledge base and competency in clinical practice
- Evidence of service development initiatives
- Experience of Clinical Governance and risk assessment/management processes
- Experience of teaching and mentorship in a clinical setting or working towards
