Job summary
As a Diabetes Nurse, you will provide and maintain a high standard of diabetes management for Type 1 & type 2 patients, which includes diagnosis, monitoring & development of individual management plans, agreeing these as appropriate with the patient & other health professionals. Specifically in Diabetes management and desirably in CHD and HF; you will work strictly in accordance with specific practice guidelines & protocols. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care. The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy, and performance.

Main duties of the job
- Working independently as a Diabetes Nurse in type 1 & type 2 management. Including managing diabetic reviews and taking appropriate action as per current guidelines
- Assisting the general practice team with monitoring, educating, and reviewing patients on the diabetic register
- Provide general and specific health screenings to the practice patients within agreed protocols with referral to other members of the primary and secondary health care team as necessary.
- Recognise health promotion opportunities and give brief, focussed lifestyle advice using motivational interviewing techniques which may include weight loss, diet, exercise, smoking cessation
- Assist with nurse-run diabetic clinics and carry out clinical measurements and near patient testing in line with NICE guidance and Quality and Outcome Framework indicators.
- Recognise signs and symptoms that may indicate undiagnosed conditions that may exist alongside diabetes.
- Work in accordance with Practice protocols to ensure the safe and effective management of patients with Type 1 or Type 2 Diabetes

About the Practice/Department/Team
Lakeside St Neots is in a lovely town location with around 21,000 patients. We are a diverse and dynamic team of GPs, Pharmacists, Physios, Social Prescribers, Practice Nurses, Health Care Assistants, Nursing Associates, Emergency Care Practitioners and ANPs.
