Job summary

An exciting opportunity has come up to recruit a Care Coordinator within the Rockingham Forest Primary Care Network (RFPCN).

The Care Coordinators role is to work closely with general practice teams, patients, carers, and the wider multidisciplinary team (MDT) to actively support patients especially those who are more vulnerable or dealing with long-term conditions, complex needs, or at risk of hospital admissions. In your daily work, you'll focus on providing personalized, coordinated, and holistic care to help patients navigate health and social care systems more smoothly and achieve better outcomes.

Main duties of the job

You will work closely with the patient and their clinician to co-ordinate patient healthcare and direct them to the appropriate service to ensure that they get the most suitable care.

You will:

  • Support the design and delivery of personalized care plans
  • Coordinate care across services and support MDT meetings
  • Work with patients, families and carers to improve health outcomes
  • Facilitate referrals, appointment scheduling, and shared decision-making
  • Visit patients in community settings including homes and care homes
  • Help track patient progress using clinical tools (e.g. SystmOne)

About the Practice

Rockingham Forest PCN is a large, forward-thinking Primary Care Network made up of 3 practices across Corby, Kettering, and Oundle.

This role is based at the Lakeside surgeries in Corby, Kettering, and Oundle.

  • Experience of working with healthcare professionals and/or previous experience within the NHS or social care setting
  • Experience coordinating with multiple stakeholder or individuals to meet specified outcomes
  • Experience of data collection and providing monitoring information to assess the impact of services
  • Experience of partnership / collaborative working and of building relationships across a variety of organisations including the voluntary sector
  • Experience of supporting people

Desirable

  • Experience providing advice/signposting
  • Experience of using clinical systems such as SystmOne
  • Experience of supporting service improvement

Qualifications

  • NVQ Level 2 equivalent in a relevant occupational qualification i.e. Health & Social Care
  • Good level of education with GCSE Math and English Grade C or above (or equivalent)
