The role

At Lakeside Healthcare we are looking to recruit a fulltime Safeguarding Nurse to come and join our busy safeguarding team based at Forest Gate Medical Centre in Corby.

At Lakeside Healthcare we work in partnership with countywide professionals to ensure that safeguarding adults and children is high on the agenda. Main duties of the job

Through liaison, research and dialogue identify and collate relevant health information to add to multi-disciplinary risk assessment discussions and Multi-Disciplinary Team Meetings (MDTs). Evaluate and Analyse health information in the MDT process, working in partnership with other professionals to identify a clear plan for each individual child and family.

Under supervision of the Head of Safeguarding, support the safeguarding care co-ordinators to provide timely and accurate case conference reports and health safeguarding reports within the required timescales. Ensure that relevant and proportionate medical information is shared within the required timescales to form part of the child protection multi-agency case conference process. Attend Initial Child Protection and Review Child Protection Case conferences on behalf of the LH and PCN practices as required.

Attend key meetings as required, such as the practice Multi- Disciplinary Team Meetings (MDTs). Risk assess the needs of the child and family, discuss highly complex safeguarding information, ensuring a sound rational and plan of action for all cases discussed is recorded within SystmOne.

To carry out the role and responsibilities of a Safeguarding Nurse for the Lakeside Healthcare Group (LHG) Safeguarding Team and on behalf of the Primary Care Network Practices

Job Responsibilities

The Safeguarding Nurse will work as a dynamic member of the safeguarding team to ensure a high-quality safeguarding service is delivered to all Lakeside Healthcare (LH) staff, patients and across the Primary Care Network (PCN) Practices. The individual Safeguarding Nurse will lead multi-disciplinary meetings (MDTs) across the LHG and for PCN practices, planning and assessing appropriate care for all children and families. They will work alongside safeguarding partners within Northamptonshire, Cambridge and Peterborough, Lincolnshire and Rutland to improve outcomes for vulnerable children, young people and adults. This will include providing advice, support and relevant information to a multitude of professionals, enabling the safeguarding team to assess the most appropriate plan of action for each individual case reviewed. The Safeguarding Nurse will deputise for the Head of Safeguarding, when required, to ensure the safeguarding team continue to deliver all the key objectives, statutory functions and CQC requirements in respect of LH and PCN practices. The safeguarding Nurse will be responsible for supporting all Doctors and Nurses to complete their statutory duty within the safeguarding arena. The safeguarding Nurse will develop and maintain effective working relationships with all colleagues.

With support of the Head of Safeguarding and allocated Safeguarding GPs, communicate complex and sensitive information to a variety of professionals to ensure consistent advice is provided to professionals on best practice in safeguarding children and adults.

Contribute to the development of Safeguarding Children and Adult policies and guidance within LHG.

Ensure patient records are maintained by the team, complying and adhering to the practice record keeping systems (policy) within LH. Offer concise and constructive feedback to all members of the Safeguarding team to ensure accurate and timely records are available to all staff within LH and PCN Practices.

Act as an advocate for children and young people, raising awareness of their needs and actively promoting the philosophy that the interests of children are paramount.

Liaise with Social Workers to obtain informed and written consent prior to any information being shared with professionals outside of LH, unless this meets the requirements within LH or Partnership safeguarding Childrens and Adults policies.

Support the Safeguarding Care Co-ordinators to ensure all reports and requests for health information are provided in a timely and professional manor. Ensuring these are updated and allocated to all members of the safeguarding team.

Contribute to the collection of data for safeguarding metrics that accurately reflects the safeguarding team activity and support with monthly and annual safeguarding reports for LH.

Participate in audit activity as required within the safeguarding team in relation to both Adult and Child data for LH and PCN practices.

Promote appropriate and effective communication with all health professionals and multi-agency staff. Challenge decisions when appropriate and escalate concerns through the required channels for patients of LH and PCN practices.

Manage and maintain the safeguarding information database, to ensure cases are reviewed in line with agreed timescales.

Provide support and guidance to the Safeguarding Care Co-ordinators in relation to any safeguarding concerns.

Contribute to the delivery of safeguarding information and ongoing L2 and L3 safeguarding training for LH staff and PCN practices.

Support the Safeguarding team to deliver additional work or projects as required by LH.

Ensure that practice standards meet the RCGP guidance and NMC code.

Ensure yearly individual revalidation and registration in line with the NMC.

The Safeguarding Nurse must have the ability to travel to and from work within a variety of GP surgeries covered by LHG and PCN practices.

Personal Development

Receive supervision from the Head of Safeguarding on a regular planned basis in recognition to the potential impact of working in safeguarding on self and others. Seek support and help where necessary.

Attend monthly 1-1 meetings with the Head of Safeguarding to develop clear actions plans of individual work and progress within the team.

Participate in annual appraisals, coinciding with the NMC requirements of revalidation and re-registration.

Work with the Head of Safeguarding to develop an individualised education plan to enhance knowledge and progression within the safeguarding team.

Attend countywide training sessions as required or as identified within the individualised educational plan.

To be responsible for the self-development of skills and knowledge linked to safeguarding processes within all areas covered by LHG and PCN practices.
