Job summary

The purpose of the post is to deliver good quality Leadership to your team. Whilst bringing your clinical skills to provide, high quality primary care services t0 the practice population. Utilising skills and experience, the post holder will provide advanced interventions and influence development of services.

Using evidence-based knowledge, the post holder will deliver a high standard of care, making professionally autonomous and accountable decisions. The post holder will work within their professional boundaries to demonstrate safe clinical decision-making and cost-effective, expert care for patients within the practice.

The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective nurse advocacy and leadership.

Main duties of the job

  • Provide effective Leadership to the Nursing/AHP team, motivate, support and develop them.
  • Manage Annual appraisals and 1:1s, setting clear objectives.
  • Chair Regular team meetings locally ensuring active contribution and participation, and attend Lead Nurse/AHP meetings within the Lakeside Healthcare.
  • Ensure effective communication between teams, ensuring teams are working within set standards.
  • Initiates health promotion activities with individuals, patients and carers.
  • Actively involved in the development and implementation of service developments and initiatives.
  • Participates in risk assessment and promotes the safety and well-being of patients, staff and any other persons at all times and assist in ensuring a safe work environment.
  • Ensure that all complaints, untoward incidents, accidents and areas of clinical risk are identified and reported to the lead GP, Nurse Manager or Practice Manager in accordance with the organisational policy and expected professional practice.

JOB SUMMARY:

Supporting the overarching Lakeside Healthcare Partnership strategy, the Lead nurse/ACP/ACP is responsible for leading and managing the Nursing Teams and responsible for all aspects of performance related to their clinical teams and specialty clinical services.

Reporting to the Non-Executive Director and in conjunction with the Head of Nursing & Allied Health Professionals, Medical Directors and local Operational Teams, the Lead nurse/ACP/ACP plays a crucial role in delivering day-to-day services, leading service improvement and development programmes.

Acting as a role model, the Lead nurse/ACP will deliver high clinical and operational performance standards by developing and supporting the Nursing Teams in all aspects of clinical and professional practice.

As a professional and experienced Lead nurse/ACP the post holder will also: -

  • Motivate, develop and empower staff, leading by example and ensuring that local standards of care are delivered, monitored and reported within the Quality Governance framework, including CQC & Risk Management, and ensure that operational performance is consistent with the expectations of the Lakeside Healthcare Partnership.
  • Be instrumental in assisting Lakeside Healthcare to achieve its service development plans, performance targets and organisational strategy including QOF.
  • In conjunction with the multi-disciplinary team, ensure that there is an embedded philosophy and practice of continuous improvement by encouraging innovation and creativity; undertaking ongoing assessment and review of clinical and nursing practice standards, patient pathways and experience as well as operational performance and outcomes.
  • Be clinically visible and directive, leading the clinical nursing teams and ensure consistent and timely decision-making in relation to clinical and operational management of the nursing team.

Key Performance Areas Management responsibilities:

  • Enable the nursing team to work effectively and provide a high-quality service to patients and to work effectively with other agencies.
  • Monitor and advise on the effective use of physical and financial resources for/by the nursing team.
  • Enable the nursing team to apply and improve clinical governance and risk management systems.
  • Lead and monitor the development and implementation of policies, standards and guidelines across the Lakeside Healthcare nursing team.

Quality Assurance, Risk Management & Performance

In partnership working with the Practice Leadership Team, the Lead nurse/ACP will be jointly responsible for locally delivering the following: -

  • Ensuring all care and services are delivered to the highest standards at all times and across the CQC domains, Safe, Effective, Caring, Well-led and Responsive.
  • Continually monitor the quality of care and make appropriate and timely decisions to ensure the highest standards are maintained at all times.
  • Work collaboratively with the broader MDT to ensure delivery plans are compatible and produce more efficient, patient centric and quality focused.
  • Work closely with nursing teams to continually review and assess efficacy or roles and responsibilities, ensuring alignment to overall local and organisational strategy.
  • Contribute towards and support staff with re-design projects, providing experience and advice.
  • Ensure that the nursing workforce has the right numbers of staff with correct skills and expertise to deliver high quality care in the most effective and efficient way.
  • Work closely with the NED to maintain the Risk Register and undertake investigations of incidents as required.
  • Ensure management information provided is utilised to inform all resultant actions focusing on enhancing the quality of care and patient experience.
  • Contribute to financial performance delivery in line with the Lakeside Healthcare agreed financial plan; oversee budgets relating to the Nursing Teams, ensuring that they are managed correctly and identify opportunities for cost improvements or improved resource management.
  • Ensure compliance with statutory requirements including quality schedule, standing financial instructions and Lakeside policies and procedures.

  • Work closely with the HoN and NED to help lead, deliver and embed the Lakeside Nursing Strategy.
  • Be conversant with national professional nursing agenda and provide local professional nursing leadership; this includes line management, mentorship, advice and support, monitoring and compliance, including adherence to Lakeside Protocols, Procedures, Guidelines and Policies.
  • Be available and accessible to patients, public and staff to discuss issues of concern and take appropriate action where indicated.

Workforce Management

In partnership working with the Practice Leadership Team, the Lead nurse/ACP will be jointly responsible for delivering the following: -
