Job summary

As a specialist Nurse, you will provide and maintain a high standard of chronic disease management, which includes diagnosis, monitoring and development of individual management plans and agreeing these as appropriate with the patient and other health professionals. You will work strictly in accordance with specific practice guidelines and protocols, using Ardens templates and the patient digital platforms wherever possible.

The post holder will work collaboratively within the multi-disciplinary general practice team, to meet the needs of patients, supporting the delivery of policy and procedures. The post holder will provide specialist assessments and reviews of health care needs, the optimisation of care where co-morbidity exists, maximising wellbeing, self-empowerment, and quality of life.

Main duties of the job

  • Be proactive in chronic disease, QoF and Enhanced Service management, including health promotion, processing pathology, workflow, tasks and running audits as required.
  • Utilise Ardens templates to complete thorough and concise annual chronic disease reviews and champion new digital platforms to maximise their potential for efficiency savings.
  • Act as a mentor to junior/less experienced staff. Provide education, support, specialist advice and guidance to colleagues.
  • Refer patients to other services/agencies and members of multidisciplinary teams as necessary e.g. Secondary Care, DESMOND, WMP, NDPP, social prescribers, care coordinators, health and wellbeing coaches.
  • Recognise signs and symptoms that may indicate undiagnosed long term conditions and utilise relevant referral pathways.

Job responsibilities

The purpose of the post is to deliver safe, high quality primary care nursing services to the practice population. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care. The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy and performance.

DUTIES & AREAS OF RESPONSIBILITY

Job Responsibilities/Principal Duties to include:

  • Responsible for management and running of long-term condition clinics in your area of specialty e.g. diabetes, respiratory, in line with NICE guidance and Quality and Outcome Framework.
  • Recognise health promotion opportunities and give brief, focused lifestyle advice using motivational interviewing techniques which may include:
Alcohol screening

Smoking cessation

Weight management

Sexual health

  • Complete annual long-term condition reviews for patients using Ardens templates.
  • Complete medication reviews and change when necessary according to results.
  • Recognise signs and symptoms that may indicate undiagnosed long term conditions e.g. poor wound healing, winter cough in smokers and also factors that may indicate mental health problems and use relevant referral pathways.
  • Work in accordance with Practice protocols to ensure the safe and effective management of patients with Chronic Diseases.
  • Review patients who present to the service on the day needing an acute review.
  • Be proactive in QoF and enhanced services.
  • Process pathology, workflow and tasks.
  • Provide advice and guidance to colleagues.
  • Complete audits as required.
  • Liaise with other members of the multidisciplinary teams and refer patients on when required e.g. social prescribing team, health and well-being coaches, specialist hospital teams.
  • Proactive in improving and influencing the patient care pathway/ journey.
  • Effective communication with patients, carers and all members of the MDT.
  • Effective prioritisation and completion of daily work in line with clinical / patient need and practice priorities.
  • Help supervise and provide training to colleagues/students in your specialist area.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.

Training and personal development:

  • Actively participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.

  • If it is necessary to expand the role to include additional responsibilities, full training will be given.

Other administration and professional responsibilities:

  • Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards.
  • Maintain professional registration and practice in accordance with NMC requirements.
  • Attend and participate in nurse meetings.
