Job summary

Do you have experience in managing diabetic patients within a Primary Care environment? Or are you an experienced Practice Nurse who has expanded their skills and has experience in managing diabetes?

We are recruiting an enthusiastic Diabetic Specialist Nurse to work across the LHG (Lakeside Healthcare Group) in Northamptonshire (main base will be Headlands, however, some cross-site working at our other sites at Corby and Oundle may be required). All practices are active members of their local Primary Care Network and share the same group Group Lead Nurse and Chief Medical Officer.

As a Specialist Nurse, you will provide and maintain a high standard of chronic disease management, which includes diagnosis, monitoring and development of individual management plans and agreeing these as appropriate with the patient and other health professionals.

Main duties of the job

The post holder will work collaboratively within the multi-disciplinary general practice team, to meet the needs of patients, supporting the delivery of policy and procedures. The post holder will provide specialist assessments and reviews of health care needs, the optimisation of care where co-morbidity exists, maximising wellbeing, self-empowerment, and quality of life.

The post holder will be expected to:

Responsible for managing and running long-term condition clinics for your speciality

Be proactive in QoF and enhanced services management by completing annual reviews using Ardens templates and patient digital platforms

Review patients who present to the service on the day needing an acute review

Process pathology, workflow, tasks and assisting with audits as required

Refer directly to other services/agencies and members of multidisciplinary teams when needed e.g social prescribers and health and well-being coaches

Supervise and provide training to other members of staff

Kettering is a market and industrial town in Northamptonshire, with a population of around 63,675. It has a growing commuter population due to direct train links to London.

Lakeside Healthcare at Headlands Surgery has 10,031 patients registered and is rated as GOOD with CQC. Working within the NMC Code of Conduct and professionalboundaries, the post holder will deliver primary care health interventions and care. Thepost holder will support service development, the safe delivery of policy and procedures,and demonstrate effective professional nurse standards, advocacy and performance.

The role of the DSN is to facilitate the care and management of patients with type I & IIdiabetes; both in general practice and at home. The aim is to support independence andto help people self-manage their diabetes more effectively. The DSN is a specialist whoworks exclusively with people with diabetes. The role ranges from diagnosis and healtheducation, to supporting patients with complex health needs associated with diabetes.The DSN has the ability to refer to specialist teams where appropriate. The DSN is acompetent autonomous practitioner who is able to case manage, initiate and reviewmedications, including insulin. They are confident to seek specialist / consultant advicewhere appropriate.

DUTIES & AREAS OF RESPONSIBILITY

Job Responsibilities/Principal Duties to include:

Working collaboratively with the multi-disciplinary general practice team andother healthcare professionals to promote integrated pathways of care to meetthe needs of patients.

Effective communication with patients, carers and all members of the MD.

Effective prioritisation and completion of daily work in line with clinical/patientneed and practice priorities.

Maintenance of accurate and legible patient records in line with professional andorganisational requirements this includes contemporary paper records andelectronic patient record system.

Actively participates and contributes to team meetings.

Initiates health promotion activities with individuals, patients and carers.

Participates in clinical audit and the implementation of changes to ways ofworking and/or clinical practice as a result of findings, recommendations andaction plans.

Participates in risk assessment and promotes the safety and well-being of patients, staff and any other persons at all times and assist in ensuring a safe workenvironment.

Proactive in improving and influencing the patient care pathway/journey.

Responsible for the security, care and maintenance of equipment ensuringstandards of infection control and safety are maintained.
