Job summary

Do you have experience in managing one or more long-term condition within a Primary Care environment? Or are you an experienced Practice Nurse who has expanded their skills and has experience in managing long term conditions?

We are recruiting an enthusiastic Chronic Disease Specialist Nurse to work across LHG (Lakeside Healthcare Group). All practices are active members of their local Primary Care Network and share the same group Group Lead Nurse and Chief Medical Officer.

As a Specialist Nurse, you will provide and maintain a high standard of chronic disease management, which includes diagnosis, monitoring and development of individual management plans and agreeing these as appropriate with the patient and other health professionals.

Main duties of the job

The post holder will work collaboratively within the multi-disciplinary general practice team, to meet the needs of patients, supporting the delivery of policy and procedures. The post holder will provide specialist assessments and reviews of health care needs, the optimisation of care where co-morbidity exists, maximising wellbeing, self-empowerment, and quality of life.

The post holder will be expected to:

  • Responsible for managing and running long-term condition clinics for your speciality.
  • Be proactive in QoF and enhanced services management by completing annual reviews using Ardens templates and patient digital platforms.
  • Review patients who present to the service on the day needing an acute review.
  • Process pathology, workflow, tasks and assisting with audits as required.
  • Refer directly to other services/agencies and members of multidisciplinary teams when needed e.g social prescribers and health and well-being coaches.
  • Supervise and provide training to other members of staff.

The purpose of the post is to deliver safe, high quality primary care nursing services to the practice population. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care. The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy and performance.

DUTIES & AREAS OF RESPONSIBILITY

Job Responsibilities/Principal Duties to include:

  • Responsible for managing and running long-term condition clinics in your area of speciality e.g. diabetes, respiratory.
  • Recognise health promotion opportunities and give brief, focussed lifestyle advice using motivational interviewing techniques which may include:

Alcohol screening

Smoking cessation

Weight management

Sexual health

  • Complete annual long-term condition reviews for patients using Ardens templates.
  • Complete medication reviews and change when necessary according to results.
  • Recognise signs and symptoms that may indicate undiagnosed long term conditions e.g. poor wound healing, winter cough in smokers and also factors that may indicate mental health problems and use relevant referral pathways.
  • Work in accordance with Practice protocols to ensure the safe and effective management of patients with Chronic Diseases.
  • Review patients who present to the service on the day needing an acute review.
  • Be proactive in QoF and enhanced services.
  • Process pathology, workflow and tasks.
  • Provide advice and guidance to colleagues.
  • Complete audits as required.
  • Liaise with other members of the multidisciplinary teams and refer patients on when required e.g. social prescribing team, health and well-being coaches, specialist hospital teams.
  • Proactive in improving and influencing the patient care pathway/ journey.
  • Effective communication with patients, carers and all members of the MDT.
  • Effective prioritisation and completion of daily work in line with clinical / patient need and practice priorities.
  • Help supervise and provide training to colleagues/students in your specialist area.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.

Training and personal development:

  • Actively participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.
  • Maintain professional portfolio and ensure professional requirements are met.
  • If it is necessary to expand the role to include additional responsibilities, full training will be given.

Other administration and professional responsibilities:

  • Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards.
  • Maintain professional registration and practice in accordance with NMC requirements.
  • Attend and participate in nurse meetings.
