Main duties of the job

Dedicated visiting team, reducing pressure on GPs
Team of 2 Clinical Pharmacists, 9 nurses and 4 HCAs supporting and delivering minor illness, mental health, chronic disease and treatment room services
Friendly team of 6 Partners and 3 Salaried GPs, meaning support is always available
Long established training practice, providing opportunities for career development
Established pharmacist team supporting medication reviews, chronic disease and clinical correspondence
Large multi-disciplinary team including Health &Wellbeing Coach and Safeguarding
