We are looking for someone to work one day (8 hours) a week.

Main duties of the job

As an Advanced Nurse Practitioner (ANP) you will be well supported at all times by an onsite GP. You will be expected to be an experienced clinician who, acting within your professional boundaries, will provide same day care for patients. This will include telephone triage, initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. You will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills. You will demonstrate critical thinking in the clinical decision-making process.

This will include telephone triage, initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. KEY RESPONSIBILITIES

Clinical Care

  • Act as Advanced Nurse Practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
  • Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat or refer patients
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-
  • based treatment methods into a management plan
  • Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.
  • Work with patients and their carers in order to support compliance with and adherence to prescribed treatments
  • Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members
  • Ensure that patients receive high quality clinical care, delivered in a timely manner.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs
  • Ensure complete and accurate documentation of each and every patient contact.
  • Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met.
