A Health Care Support Worker works alongside a team with the Care Home Team, 18 care homes and wider PCN teams.
We have a permanent vacancy for a Health Care Support Worker to join our large, supportive, busy and friendly team. The successful candidate will play an important role in complementing our current Care Home Team and other members of the PCN multi-disciplinary team.
Main duties of the job
We are seeking an experienced Health
Care Support Worker who has a passion for care of the elderly and experience of
working in the community. They must be able to work autonomously in their
clinical work delivering reactive and proactive care to care home residents. We
are looking for an individual who will help support our established Care Home
team, alongside ANPs & Care Co-Ordinators. Other members of the wider PCN
team will support you in this.

Network (PCN) is looking to expand their Care Home Multidisciplinary Team. We
are recruiting an experienced Health Care Support Worker for our care home team in both the support of
patient care and work in line with the new Network Contract Directed Enhanced
Service (DES) Specification The post holder will be delivering care and
developing the service according to the principles of the Enhanced Heath in
Care Homes framework and working closely with practices to deliver outstanding
patient care.
The post holder will work closely with 18 Care Homes and the 6
practices, Ancora Medical Practice, Ashby Turn Primary Care Centre, Cedar
Medical Practice, Cambridge Avenue Medical Centre, Kirton & Scotter Surgery
and West Common Lane within the PCN and other local organisations.
To Carry out clinical assessments on residents.
- Ability
to carry out a Blood pressure checks on a patient and be able to act on the
results.
- Ability
to carry out venepuncture or be willing to undertake training
- Ability
to follow protocols and work within scope of competency and feed back to
registered nurse
- Ability
to use own initiative and work without close supervision
- Able
to listen effectively and demonstrate a sympathetic/ sensitive approach. Able
to speak effectively to the wider MDT/ patients and families

a patient

contribute to and embrace the spectrum of clinical governance

develop yourself and the role through participation in training and service
redesign activities
- To
attend a formal appraisal with their manager at least every 12 months. Once a
performance/training objective has been set, progress will be reviewed on a
regular basis so that new objectives can be agreed.
- To
contribute to public health campaigns (e.g. flu/Covid clinics) through advice
or direct care
- To
maintain a clean, tidy, effective working area at all times
- Support
delivery of QOF, incentive schemes and other quality or cost effectiveness
initiatives
- Undertake
any tasks consistent with the level of the post and the scope of the role,
ensuring that work is delivered in a timely and effective manner
- Duties
may vary from time to time without changing the general character of the post
or the level of responsibility
- Work
within a team & own initiative to work independently
- Attend
