Job summary

The post holder will be working within a multidisciplinary team of healthcare professionals to provide a high-quality service to the patients of Tower Family Healthcare by working closely with our established Practice Pharmacy team and Prescriptions team, to support the development of high quality, safe and cost-effective prescribing in the practice, utilising and reviewing prescribing systems, implementing processes to ensure evidence-based practice is embedded in patient care, identifying and streamlining medication review process, assisting with improving medicines optimisation and carrying out medication audits.

Main duties of the job

Contributing to the provision of safe and effective pharmacy services

Undertaking medicines reconciliation in line with practice protocols and standard operating procedures to improve the efficiency and effectiveness of prescriptions and repeat prescriptions

Participating in medication audits

Providing advice and information to all staff and patients to support the improvement of prescribing safety, quality, cost effectiveness and the appropriate use of medicines to get the best outcomes for patients

Assisting in the monitoring and management of prescribing budgets and the preparation of evidence-based resources and information that support cost-effective prescribing

Monitor local and national prescribing targets, reviewing prescribing patterns, financial changes, and adherence to local guidelines and formularies

Taking responsibility for contributing to all aspects of the day-to-day management of the working environment including communication, data accuracy and management, keeping practice areas tidy and helping to achieve a supportive, positive culture across the practice team

To support and promote a safe working environment in accordance with Health & Safety and Infection Control policies

To maintain clear, accurate and contemporaneous computerised records of all patient contacts and respect and protect patient confidentiality and information

To assist the practice in managing the introduction of new drugs and drugs that have been withdrawn

We are a large GP practice with a multi skilled clinical team that works over four sites. Our sites are located in Bury town centre (Minden), Greenmount, Tottington and in Radcliffe (Spring Lane).

We provide services to around 48,000 registered patients, thats a quarter of the population of Bury, and our patients can access services across all four of our sites.

Our clinical team is diverse including GPs, a Practice Paramedic, Pharmacists, Advanced Clinical Practitioners, Advanced Nurse Practitioners, Practice Nurses, Nurse Associates, a Physician Associate and highly skilled Health Care Support Workers, working closely with a range of additional roles including an Acute Visiting Service, Mental Health Practitioners and First Contact Physiotherapists.

To work closely and collaboratively with other members of the practice management team to achieve high quality, safe and evidence-based patient care consistently across all sites of Tower Family Healthcare

To work collaboratively with both clinical and non-clinical staff to deliver seamless care to the patients of Tower Family Healthcare.

To work collaboratively with the ICBs Medicines Optimisation Team to support the implementation of medicine related ICB initiatives within the practice.

To liaise closely with local community pharmacists, community nurses, care home staff, hospital pharmacists and other care providers, as necessary to ensure accurate exchange of information/clarification of queries regarding medicines.

To communicate effectively to a variety of healthcare professional, patient, and community groups. This may involve small group sessions, public speaking or making presentations as appropriate.
