Job summary

Stockbridge Village Health Centre is a long established practice in a residential area that has had a lot of changes recently. We are changing and building new teams to cope with the changing face of primary care and are looking for an enthusiastic personable ANP to be part of the nursing team to continue the patient focused work of the practice.

Main duties of the job

A requirement for the role is to be registered with the NMC, be an independent/supplementary nurse prescriber and hold an MSC/Post graduate diploma or equivalent.

Main duties of the job

  • Make professionally autonomous decisions for which he/she is accountable
  • Provide a first point of contact for patients to establish a diagnosis and management plan
  • Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings / reports
  • Prescribe safe, effective and appropriate medication as defined by current legislative framework in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
  • Refer patients directly to other services / agencies as appropriate
  • Work with multi-disciplinary team within the practice, and across the wider NHS, to promote integrated and seamless pathways of care
  • Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.
  • Help develop and set up new patient services and participate in initiatives to improve existing patient services
  • Make home visits to those housebound patients
  • Assessing patients in surgery and liaising with GPs and outside agencies
  • Provide assistance to GPs and treating patients referred to you

The post holder will be an experienced, caring, Advanced Nurse Practitioner, who can demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries. The Advanced Nurse Practitioner will act as an autonomous advanced practitioner who is able to work independently and in conjunction with other members of the clinical team.

You will be able to demonstrate safe, competent clinical decision-making and deliver expert care, including assessment, diagnostic skills, treatment and evaluation of patients who wish to be seen by a healthcare professional; presenting with minor illness and/or their continuing medical and nursing needs.

The Advanced Nurse Practitioner will work autonomously and be accountable for own professional actions. Working at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.

The key duties are:

To carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.

To assess, triage, diagnose and treat patients face to face accordingly, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams.

To request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.

To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly, explaining medical problems during surgery consultations.

As an independent prescriber, to review and prescribe medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.

To ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations, embarking upon further training if required.

To ensure patient confidentiality, respect, and dignity is adhered to at all times.

Makes decisions where precedents do not exist, where appropriate without recourse to others e.g. direct referral of patients to Hospital Consultants without prior reference to other medical colleagues.

Advise and support others where standard protocols do not apply.

Works within professional guidelines and codes of conduct.

Works within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
